Use of long autologous nerve grafts in brachial plexus reconstruction: factors that affect the outcome

Abstract: The results of a series of distal brachial plexus target reinnervations with long grafts is presented and analyzed. According to them, time from trauma to surgery and an adequate postoperative rehabilitation are important predictors of outcome.

“…Variables like patient age, gender, time from trauma to surgery, and the extent of the lesion were recorded. Two comparison groups were constructed, with 20 patients in each group (total = 40 patients) all sequentially recruited over the same time span, starting January 1,2006. Patients in group #2 were between 20 and 29 yr old, inclusive, while group #3 consisted of patients 30 yr old or older at the time of surgery.…”

“…[11171820] Body mass index (BMI), which reflects a patient's weight relative to height, has only recently been reported as yet another prognostic factor, with its influence appearing not to be as strong as the timing of surgery. [93132]…”

“…Several studies 19,[27][28][29][30] have provided convincing evidence supporting the use of long grafts. Such grafts have been documented to facilitate the growth of regenerating nerve fibers toward a more selective target, avoiding axon misrouting and bypassing possible areas of distal double crush.…”
