Use of Inoue balloon dilatation method for treatment of Cor triatriatum stenosis in a child

Abstract: An 8-year-old girl was referred for evaluation of a heart murmur and progressive dyspnea with exertion. Transthoracic echocardiogram revealed Cor triatriatum with severe stenosis at its orifice. The resting transmembrane CW Doppler velocity was 2.3 m/sec. Cardiac catheterization and hemodynamic assessment were performed and demonstrated a marked increase in pulmonary capillary wedge and pulmonary artery pressures. The stenosis was successfully dilated with an Inoue balloon using a transseptal technique. This i… Show more

“…Surgery has been advocated as early as the neonatal period, 18 and balloon dilation may be successful in older children. 19 Some patients remain asymptomatic until adulthood 20 or may present with sick sinus syndrome in advanced years. 21 In adults, clinical features on presentation can mimic those of mitral stenosis because of the obstructive properties of the membrane 22 ; they include dyspnea with increased pulmonary capillary wedge pressure (PCWP) on exertion.…”

“…23 An open surgical approach using cardiopulmonary bypass is preferred over rarely performed percutaneous interventions for treatment of this type of cardiac anomaly. 34 The role of percutaneous balloon dilation in managing this condition remains to be determined. 4,35 If emergency surgery is indicated during pregnancy, the anesthesiologist must follow the same hemodynamic principles used for intraoperative management of mitral stenosis (avoid tachycardia, maintain euvolemia, avoid hypotension [lateral tilt], and avoid an increase in pulmonary vascular resistance from hypoxia, hypercapnia, or certain drugs).…”

“…4 En nuestro caso, la auriculotomía izquierda fue elegida debido a que la AI se encontraba casi anterior, por lo tanto, fue más accesible quirúrgicamente que su homóloga contralateral. También se ha descrito el tratamiento del CT mediante cateterismo en niños, 11 pero no en neonatos ni lactantes.…”
