Use of Human Fibrin Glue (Tissucol) Versus Staples for Mesh Fixation in Laparoscopic Transabdominal Preperitoneal Hernioplasty

Abstract: The use of Tissucol provides distinct advantages in laparoscopic treatment of inguinal/femoral hernias compared with conventional TAPP, including a lower incidence of postoperative neuralgia and an earlier resumption of physical and social activities.

“…With a median follow-up of 1.2 years (84 % FU), there was no significant difference in chronic pain (20 vs. 13.2 %; P = 0.418); no recurrences were seen in any group [41]. A comparable study was done in TAPP [42]. The mesh, cut with a slit for the spermatic cord structures, was fixed with the same endoscopic Hernia Stapler (three clips at the Cooper ligament and pubic tubercle, lateral to the internal ring and both slits fixed) or with 1 ml of fibrin sealant anterior and posterior to the mesh).…”

“…Despite the previous non-absorbable tacks which were associated to a high incidence of nerve or muscular entrapment causing pain, para-esthesia and other chronic complications, the new U-shaped absorbable device may reduce pain or not result in it at all (15)(16)(17)(19)(20)(21)(22)(23)(24). Conversely, the tacks can be used to close the peritoneum reducing operating times and technical difficulties.…”

“…In our experience, the TAPP procedure operating time has progressively improved accordingly to surgeon's and staff experience with an average operative time of 40min in bilateral hernias. Fibrin glue has proven to be both efficient and secure in mesh fixation and currently is recommended by Several International guidelines (15)(16)(17)(19)(20)(21)(22)(23)(24).…”

“…7 Penetrating devices, such as tacks, clips, staples, or strap devices should be avoided for mesh fixation and also for peritoneal closure, because of the risk of nerve injuries and adhesions. 8,9 LeBlanc 10 reported tack hernia as one of the complications after using tacks for mesh fixation or peritoneal closure in lapa roscopic hernia repair. The peritoneal incision should be noninvasively approximated, for instance, using an absorbable, whose ends get fixed with absorbable clips.…”
