Use of Foley catheter in control of internal carotid hemorrhage during endoscopic endonasal surgery

Abstract: Internal carotid artery (ICA) injuries during endoscopic endonasal surgery (EES) are rare life-threatening events. We describe a technique to manage ICA injuries based on the use of Foley catheters.A 26-year-old female underwent endoscopic transnasal trans-sphenoidal removal of pituitary adenoma. Cerebrospinal fluid leak occurred 4 days postoperatively. During repair procedure, accidental injury of ICA occurred. Emergency nasal packing through positioning of four Foley urologic catheters was successfully perfo… Show more

“…In several cases, initial packing was successful with Merocel, oxycellulose, or Gelfoam [ 1 ]. Successful balloon packing of the ICA lesion has also been reported [ 1 , 26 ]. In addition, bipolar coagulation was used successfully to manage bleeding from smaller ICA perforations [ 1 ].…”

“…Minor bleeding episodes can be addressed using bipolar coagulation for hemostasis, 1 breaks with high bleeding flow can be packed and locally pressed to stop bleeding. For instance, multiple Foley urologic catheters are packed for hemostasis, 10 hemostasis using crushed muscle packing, 11 cottonoid and hemostatic materials are packed to stop bleeding 2 . For the purpose of reducing bleeding and maintaining blood pressure, in addition to blood transfusions, anesthesiologists can also employ adenosine to induce transient hypotension, which is highly advantageous for minimizing bleeding and is both secure and efficient 8,12 .…”
