Background: Radiation skin injury (RSI) causes changes in skin temperature, but detailed information on the thermographic responses is currently lacking. We investigated thermographic patterns after radiotherapy. We hypothesized that skin temperature may be used as a diagnostic and early predictor of RSI severity. Method: All breast cancer patients received radiotherapy after unilateral postmastectomy. The contralateral supraclavicular area served as control, and the frontal thermal image of torso was taken by a thermal infrared imager weekly. We defined areas of interest on bilateral symmetrical supraclavicular area, and analyzed the difference of average and maximum skin temperature (DST average and DST max ) between them. The extent of the weekly variation in DST (DSTW) was calculated using a mathematical formula to represent a trend of skin temperature change. RSI and symptoms related to RSI were scored from baseline to 2 weeks after the end of radiotherapy. Results: Forty-one patients were enrolled in this study. In comparison to the baseline, the DST average and DST max increased significantly over time during radiotherapy (p < .05). The onset of DST increase was accompanied by the onset of radiation dermatitis, and the maximal DST also appeared at the peak of Radiation Therapy Oncology Group (RTOG) and symptom scores. Radiation dose, DST average , burning-feeling and pulling were the independent variables affecting RTOG score according to multivariate analysis (p < .001, p< .034, p< .001, p< .001). Patients with DSTW average >1.223 or DSTW max >1.114 in second week showed a late higher dermatitis score (RTOG score !2). Conclusion: This study confirmed that RSI was associated with thermographic response. Our results suggested that the follow-up observations of skin temperature during radiotherapy could provide the objective evaluation criteria and prediction methods for RSI. ARTICLE HISTORY

ABSTRAKT Wstęp: Celem doniesienia jest przedstawienie zastosowania zjawiska termoemisji w różnych dziedzinach medycyny z uwzględnieniem okulistyki. Zastosowanie tomografii: Dane w piśmiennictwie wskazują, że detekcja promieniowania podczerwonego, która jest emitowana przez różne obiekty, w tym tkanki ludzkie, może być wykorzystywana w medycynie. Termografia w medycynie najczęściej służy do oceny rozległości i nasilenia stanu zapalnego w badanej tkance na postawie zjawiska wysokiej termoemisji, czyli lokalnego wzmożonego ukrwienia i metabolizmu tkankowego. Używana jest w diagnostyce zapalenia stawów, skóry czy gałki ocznej. Wzrost temperatury obserwuje się w guzach nowotworowych takich jak czerniak oraz naczyniak. Obniżenie temperatury jako efekt zaburzeń ukrwienia stwierdza się w termografii dynamicznej w Zespole Raynauda, po zawale serca, po oparzeniach skóry oraz w niedokrwieniu siatkówki. Zaobserwowano, iż w zespole suchego oka oczy chore mają niższą temperaturę niż oczy zdrowe, co może wynikać z niższej emisji promieniowania podczerwonego przez zaburzony film łzowy. Wnioski: Termografia jest metodą nieinwazyjną, szybką oraz obiektywną, która może znaleźć w przyszłości zastosowanie w wielu dziedzinach medycyny, w tym w okulistyce.

Accurate assessment of burn wound depth and the associated healing potential is vital in determining the need for surgical treatment in burns. Infrared thermography measures the temperature of the burn wound noninvasively, thereby providing indirect information on its blood flow. Previous research demonstrated that a small, lowpriced, handheld thermal imager has an excellent reliability, but a moderate validity for measuring burn wound healing potential. A new and more sensitive version of this convenient device has become available. The aim of this study was to evaluate the validity of thermography for measuring burn wound healing potential, compared to Laser Doppler Imaging (LDI) as a reference standard. Thermal images and LDI scans were obtained from burn wounds between 2 and 5 days postburn. Temperature differences between burned and nonburned skin (ΔT) were calculated. To evaluate validity, ΔT values were compared to the healing potential categories assessed by LDI. Two receiver operating characteristic curves were created and two ΔT cutoff values were calculated to illustrate the ability to discriminate between burn wounds that heal in a time period of less than 14 days, between 14 and 21 days, and more than 21 days. Between June and October 2018, 43 burn wounds in 32 patients were measured. ΔT cutoff values of 0.6 C (sensitivity 68%, specificity 95%) and −2.3 C (sensitivity 30%, specificity 95%) were calculated to discriminate between burn wounds that heal in <14 and ≥14 days, and burn wound that heal in ≤21 and >21 days, respectively. This study shows a good validity of the feasible thermal imager for the assessment of burn wound healing potential. Therefore, we consider it a promising technique to be used for triage in local hospitals and general practices, and as a valuable addition to clinical evaluation in burn centers.
