Use of drug-eluting stents for patients with coronary heart disease

Abstract: Although DES effectively reduces restenosis, a small but significant number of patients suffer complications of in-stent thrombosis. Thus, each patient's health history should be determined before recommending DES. Patients must understand the importance of post-implantation DAT and the need for continued DAT, potentially for life, in order to reduce complications. Current recommendations advocate at least 12 months of uninterrupted clopidogrel and ASA (lifelong ASA if tolerated). With each new generation of s… Show more

“…paclitaxel, sirolimus, and the more recent zotarolimus and everolimus) [31] have been incorporated in the stent and gradually released, showed another long-term side-effect with respect to BMS, consisting in a delayed re-endothelialization after angioplasty, which leads to an excessive cell proliferation in the vascular wall and to thrombosis, thus requiring a more effective anti-platelet therapy [32].…”

“…Moreover, constrictive remodelling can also still occur in the presence of a stent in regions distal or proximal to the stented segment. The main reason is that DES were responsible for another long-term side effect with respect to BMS, consisting of delayed re-endothelialization after angioplasty and leading to excessive cell proliferation in the vascular wall and to thrombosis, thus requiring more effective anti-platelet therapy [11]. Nonetheless, DES only reduced by 50 % the need for target vessel revascularization with respect to BMS [10].…”

“…Вместе с тем, отмечено, что снижение КЖ в определенной степени связано и с неблагоприятным клиническим исходом [19]. Однако, на наш взгляд, критерием эффективности примененных процедур должно быть повышение КЖ не только при краткосрочном, но и при отдаленном наблюдении [15] [12,13,18,29]. Авторы из Нидерландов приводят результаты сравнения таких процедур реваскуляризации [26].…”
