Use of dried urine spots for screening of inborn errors of bile acid synthesis

Abstract: Background In pediatric patients with cholestasis of unknown cause, inborn errors of bile acid (BA) synthesis (IEBAS) may be considered. For the initial screening for IEBAS, clarification of the urine BA profile is essential. The transportation of urine in a frozen state via air delivery, however, is laborious and costly. This study assessed the feasibility of using dried urine spots (DUS) to establish a more convenient and affordable method of IEBAS screening. Methods We created DUS using urine samples from p… Show more

“…In 1959, DUS was used to detect specific proteins in urine, in order to diagnose diseases such as phenylketonuria [ 7 ]. The use of DUS was then adapted in the following years to search for other proteins and metabolites [ 8 , 9 , 10 , 11 , 12 ].…”

“…Affected individuals show cholestasis, hepatomegaly, splenomegaly and fat (-soluble vitamins) malabsorption, with or without pruritus. Elevated aminotransferase, hyperbilirubinemia, normal or low serum γ-glutamyltransferase levels as well as BA concentration are the characteristic lab findings in most patients with 3β-HSD deficiency [4,5] …”

“…Elevated aminotransferase, hyperbilirubinemia, normal or low serum g-glutamyltransferase levels as well as BA concentration are the characteristic lab findings in most patients with 3b-HSD deficiency. [4,5] Bile acids (Bas) are synthesized from cholesterol through a complex series of reactions. The reactions involve at least 2 pathways (classic and acidic pathways) and 17 different enzymes preferentially expressed in the liver.…”

“…During the last years, several studies determining glycosaminoglycans in urine for diagnosis of mucopolysaccharidosis using DUS have been conducted (Auray-Blais et al 2012 ; Breier et al 2014 ). Further compounds successfully being analysed in DUS include hippuric acid and creatinine (Antunes et al 2013 ) as well as the bile acid profile for detection of inborn errors of bile acid synthesis (Naritaka et al 2019 ). Also, in the scope of drug screening, several approaches using dried urine samples have been developed (Lee et al 2013 ; Michely et al 2017 , 2018 ; Otero-Fernández et al 2013 ).…”
