Use of Cardiac Markers for Monitoring of Doxorubixin-induced Cardiotoxicity in Children With Cancer

Abstract: The aim of this study was to evaluate N-terminal probrain natriuretic peptides (NT-pro-BNP), cardiac troponin T, and creatinine kinase, MB isoenzyme (CK-MB) in the determination of subclinical left ventricular (LV) dysfunction by echocardiography in patients treated with doxorubicin. We performed a cross-sectional case study of systolic, diastolic function and tissue Doppler imaging by echocardiography in children with cancer who received a certain cumulative dose of doxorubicin. Blood levels for NT-pro-BNP, c… Show more

“…Indices were calculated on data averaged from a minimum of three consecutive cardiac cycles wherever possible. The myocardial performance index 69 or the Tei index of the LV was calculated using Doppler flow intervals as follows: Tei index ¼ isovolumic contraction time þ isovolumic relaxation time)/LV ejection time. All measurements were calculated with the VEVO Analysis software (version 2.2.3).…”

“…Some authors studied the association and relationship between natriuretic peptides and diastolic LV dysfunction, and others simply checked systolic LV function. However, only a few studies 55,[63][64][65][66][67] evaluated the potential predictive value of natriuretic peptide concentrations to detect the ongoing development of cardiac dysfunction.…”

“…In the clinic, anthracycline-induced cardiotoxicity (AIC) is characterized as ventricular dysfunction with no symptoms in ≤57% of patients, and clinical heart failure after cardiomyopathy in ≤16% of the patients (3,6,7). In general, the risk of developing AIC is closely linked to the cumulative dose of anthracyclines (8,9). Furthermore, other factors such as age and gender may also contribute to the occurrence of AIC (1).…”

“…In addition, serum BNP levels are increased in patients with symptomatic LV dysfunction, indicating that BNP may be linked to the prognosis of heart failure (19). In recent years, serum BNP level was demonstrated to have relevance to the pathogenesis of AIC (9,11). BNP is secreted by ventricular cardiac cells and released into the blood in response to myocardial stretch, ventricular dilatation and increased pressure load of LV chambers, which are influenced by LV diastolic dysfunction (20)(21)(22).…”
