Deep infiltrative endometriosis (DIE) is a severe form of the disease. The median time interval from the onset of symptoms to diagnosis of endometriosis is around 8 years. In this prospective study patients were divided into two groups: cases (34 DIE patients) and control (20 tubal ligation patients). The main objective of this study was to evaluate the performance of CA-125 measurement in the menstrual and midcycle phases of the cycle, as well as the difference in its levels between the two phases, for the early diagnosis of DIE. Area Under the Curve (AUC) of CA-125 in menstrual phase and of the difference between menstrual and midcycle phases had the best performance (both with AUC = 0.96), followed by CA-125 in the midcycle (AUC = 0.89). The ratio between menstrual and midcycle phases had the worst performance. CA-125 may be useful for the diagnosis of deep endometriosis, especially when both are collected during menstruation and in midcycle. These may help to decrease the long interval until the definitive diagnosis of DIE. Multicentric studies with larger samples should be performed to better evaluate the cost-effectiveness of measuring CA-125 in two different phases of the menstrual cycle.

Un caso di ascite infiammatoria CASO CLINICOAM, donna di anni 71 anni, casalinga, nel gennaio 2007 veniva alla nostra osservazione per una progressiva distensione addominale senza dolore accusata negli ultimi quattro mesi, associata al peggioramento dei suoi abituali edemi declivi (normalmente presenti ma di minore entità) e per dispnea per sforzi lievi nell'ultima settimana, non accompagnata da alterazioni dell'alvo o febbre.La paziente era affetta da diabete mellito di tipo 2 e da cardiopatia valvolare mitralica trattata con sostituzione valvolare metallica (protesi di Biork-Shiley) nel 1982, complicata da fibrillazione atriale cronica e ipertensione polmonare, con pregressa perforazione del lembo d'inserzione (1998). Nel 1986 era stata sottoposta a colecistectomia per litiasi.La paziente riferiva allergia alla penicillina (orticaria-angioedema), intolleranza agli ACE-inibitori (tosse secca) ed allergia AbstractEven ascites appears mainly as sign of portal hypertension in patiens with liver cirrhosis, in some case depends on a different lying condition such as right congestive heart failure, peritoneal carcinomatosis or tuberculosis. In these cases, paracentesis represents the key tool for diagnosis. We report a case of cardiac ascites in a 71-years-old woman who developed in four-month an abdominal distension. Preliminary exams showed exudative ascites related to portal hypertension, a pelvic mass with caseous apparence, and inflammatory status ad an elevation of CA-125. Successive evaluation exluded peritoneal carcinomatosis or tuberculosis, underlyng a tricuspidal regurgitation. The literature on ascites has also been reviewed.

Of the biomarkers that were subjected to meta-analysis, none consistently met the criteria for a replacement or triage diagnostic test. A subset of blood biomarkers could prove useful either for detecting pelvic endometriosis or for differentiating ovarian endometrioma from other benign ovarian masses, but there was insufficient evidence to draw meaningful conclusions. Overall, none of the biomarkers displayed enough accuracy to be used clinically outside a research setting. We also identified blood biomarkers that demonstrated no diagnostic value in endometriosis and recommend focusing research resources on evaluating other more clinically useful biomarkers.
