“…In addition, the International Society for Clinical Densitometry has official statement positions regarding DXA and measurement of bone density as reference. 26,27 However, many authors have advocated that CT HU values can serve as a potential proxy to DXA given the moderate correlation between HU values and osteoporosis. 5,7,9,10,28 Another reason that HUs on preoperative CT are valuable as opposed to DXA is that it can be readily measured on the PACS, a system that nearly all modern medical centers use.…”

“…6 Multiple studies have shown that the Hounsfield units (HUs) measured on routine lumbar spine CT scans using the picture archiving and communication system (PACS) can estimate if osteoporosis is present. 2,7,10,23,26 Because of correlation between HUs and BMD, we evaluated if preoperative HUs on CT are associated with cage subsidence after single-level LLIF for degenerative conditions of the spine.…”

“…The recently published International Society for Clinical Densitometry position statement to identify who should undergo DXA before surgery was not available at the time of this study but, hopefully, it will aid in the identification of atrisk patients who may benefit from screening and preoperative BHO. 10 Our population was from a tertiary referral academic center, and the majority of patients were older Caucasian adults. Since osteoporosis has a racial, age, and sex influence, results may not apply to all populations.…”

“…9 Several proposed guidelines for primary osteoporosis treatment and secondary fracture prevention can be adopted to identify patients who should have bone density measurement and laboratory testing and, when indicated, receive medical treatment before undergoing spinal surgery. [10][11][12] We hypothesize that poor bone health is common in elective surgery patients and that many meet criteria for medical therapy. The purpose of this article was to characterize spine surgery patients who have been referred to the University of Wisconsin's BHO program.…”

“…Such images are obtained in the vast majority of patients undergoing elective spinal fusion surgery, and HUs have been correlated to BMD assessed by DXA. [8][9][10][11][12] Thus, HU determination, which can be performed by the surgeon on essentially all PACS systems, represents an opportunistic and essentially costless tool for preoperatively assessing bone quality that has the added benefit of being able to measure more specific regions of interest (ROIs; i.e., vertebral body or pedicle isthmus). An HU cutoff of < 110 in the lumbar spine has been reported to be indicative of osteoporosis.…”
