Use of Bakri balloon in post‐partum haemorrhage: A series of 15 cases

Abstract: Insertion of Bakri balloon is a simple alternative procedure in the management of PPH. It should be consider before any further surgical intervention including hysterectomy. Junior doctors and midwives can effectively apply it. It can be used during transfer or while waiting for a surgical procedure to reduce blood loss.

“…31 These values are based on portal hypertension readings, pressures required to collapse the coronary-oesophageal circuit and experimental confirmation that the oesophagus can withstand such pressures. 30 In the bladder, a pressure 'equal to the diastolic arterial pressure', approximately 75-80 mmHg, is required. 32 This 'increased intravascular pressure induces a reduction of blood circulation'.…”

“…If pressure is applied for long enough, the blood will clot and form a permanent seal'. 30 In non-uterine systems in which bleeding is successfully counteracted by balloon tamponade, relatively low ILPs are used. For example, only 25-30 mmHg is required when using the Sengstaken-Blakemore tube in the oesophagus.…”

“…The Bakri balloon, which is used as a uterine tamponade, is the only balloon product specifically designed for the control of atonic postpartum hemorrhage (PPH); however, it may lose its effect if displaced into the vagina [1][2][3][4]. The aim of the present study was to evaluate the effectiveness of a new technique for keeping the balloon inside the uterine cavity.…”

“…These advantages include being readily implementable, costeffective, and capable of eliminating major surgeries such as hysterectomy and their complications in cases with successful outcomes. 8,9 However, case selection should be carried out meticulously. It cannot be applied in cases of organ failure associated with sepsis after caesarean section or disseminated intravascular coagulation.…”
