Use of Articulating Spacer Technique in Revision Total Knee Arthroplasty Complicated by Sepsis: A Systematic Meta-Analysis

Abstract: Periprosthetic infection after total knee arthroplasty is a devastating complication, and 2-stage exchange is the standard of care in North America. Articulating spacers are effective in treating periprosthetic joint infections, but the optimal articulating spacer construct has yet to be identified. The authors performed a systematic review using MEDLINE, MEDLINE In-Process, EMBASE, BIOSIS, ClinicalTrials.gov, and Cochrane Database of Systematic Reviews. They identified 34 articles meeting inclusion criteria, … Show more

“…When full 2-stage exchange arthroplasty was performed, similar infection control was achieved between culture-negative and culture-positive conditions 71 . Articulating spacers using metal-on-polyethylene yielded increased range of motion and fewer complications with similar infection control compared with other spacers 72 . Antimicrobials within cement spacers include vancomycin 73 , ceftazidime and fluconazole 74 , and teicoplanin 75 .…”

“…The type of procedure performed also affected LOS significantly. It has been thought that articulating spacers may have a functional advantage compared to static, nonarticulating spacers [22]. To guard against the possibility that the LOS benefit from fast-track may be diminished by the type of procedure performed, we ran additional Cox regressions that included a fast-track-x-procedure interaction term to allow for this (details not shown) and found no significant evidence for such an interaction (P values of .89 univariate and >.99 multivariate).…”
