Use of antibodies recognizing cyclic citrullinated peptide in the differential diagnosis of joint involvement in systemic sclerosis

Abstract: Our data show that joint involvement is a common presenting feature of SSc. In this report, we show that anti-CCP antibodies can be detected also in patients with SSc, but they are generally less commonly present than in adults with rheumatoid arthritis (RA). Thus, the finding of high titers of anti-CCP antibodies may help to define the diagnosis of overlap syndrome SSc/RA and facilitate diagnosis and appropriate treatment.

“…In light of the high occurrence of TB in our SLE patients (13), a further possible explanation is that some of these patients may be harbouring latent TB. The 5.6% frequency of aCCP in the SSc patients is similar to the 5-15% described previously in SSc [22,23], but again we found no association with inflammatory arthritis as others have suggested.…”

“…To date there are several reports evaluating the meaning of anti-CCP antibodies in connective tissue diseases other than RA; in systemic sclerosis (SSc) 2 and systemic lupus erythematosus (SLE) 3 these antibodies are markers of erosive arthritis, as we suggest in antisynthetase syndrome. In contrast, in Sjögren's syndrome 4 this association is lacking, although anti-CCP seem to be closely associated with the occurrence of synovitis.…”
