Use of a thrombin–gelatin haemostatic matrix in endoscopic endonasal extended approaches: technical note

Abstract: The thrombin-gelatin matrix could represent a valuable tool when other haemostatic strategies are ineffective or suboptimal. It is safe and biocompatible when compared with haemostatic agents currently in use.

“…There was only 1 pseudoaneurysm from the 9 cases treated with the muscle patch. In several case reports, FloSeal, a gelatin‐thrombin hemostatic matrix sealant, has been shown to be effective in endoscopic arterial major injury management when used as a focally applied, topical packing agent . This must be used with caution, however, because it has the ability to embolize into the parent vessel.…”

“…There are no previously published systematic reviews, clinical guidelines, or RCTs pertaining to the use of nasal packing during endoscopic injury of a minor artery during ESBS. There are several case reports of the use of FloSeal as an effective agent in focally applied topical packing . In addition, Avitene packing with warm saline irrigation or Gelfoam packing has also been recommended for small arterial injuries …”

“…[11,12] Floseal can cause a hosttissue reaction and therefore carries theoretical risks, such as immunologically induced coagulopathy, IgEmediated anaphylaxis, infections and bovine spongiform encephalitis. [13] Fibrin glue does not adhere strongly to wet tissue, has little impact on active bleeding and carries a risk of infection. [14,15] In addition, use of fibrin glue is limited by the time required to obtain and prepare the product.…”

“…8,11 A concern with gelatin hemostatic medications is the theoretical risks such as immunologically induced coagulopathy, IgE-mediated anaphylaxis, and the potential for thromboembolic complications. 2,7,9,12,13 Fibrin glue does not adhere strongly to wet tissue, needs a dry application surface to polymerize, and has little impact on active bleeding. In addition, the use of fibrin glue is limited by the time required to obtain and prepare the product.…”
