Objective: This research investigates the needs and opinions of radiologists on the use of enhanced information technologies and approaches to improve the functionality of Picture Archiving and Communication Systems (PACS).Method: Six interviews were conducted in the main governmental hospital in Kuwait (AL-Sabah Hospital) with radiologists, including two senior radiologists, two junior radiologists, and two trainee radiologists undertaking the Irish radiology board.Results: The radiologists identified a number of limitations that exist in current PACS and requirements to enhance usability and functionality. However, it was the case that some of the radiologists had little knowledge about the advanced trends in PACS. Four themes emerged from the thematic analysis of the data: (1) limitations of traditional PACS; (2) Features and requirements that can increase PACS functionality; (3) web based solutions of PACS; (4) PACS in mobile phones.Conclusion: It is widely recognized that PACS has limitations. This research has identified themes that, when incorporated, will enhance the functionality of PACS and provide better quality clinical practice. This research has determined the important future trends of PACS. Primarily web based solutions and use in mobile phones. The findings from this research can be used as recommendations to vendors, for product development and medical institutes to consider when undertaking implementation of PACS and training future radiologists..

The fundamental principle for telemedicine implementation in the real world is to address the basic needs of healthcare services. The utilization of telemedicine naturally aimed to overcome distance, time, and financial constraints. Remote areas that are far from the cities and healthcare centers are the main regions that would mostly get benefit from the telemedicine program, for instance, in Indonesia, a country with a big archipelago area in South-East Asia. The primary healthcare center in this country is commonly available, however, the facilities and health workers are still limited. The health services are being centralized in big cities, and thus, the rural areas are far left in the context of healthcare services. Telemedicine could bring both standardized and specialized healthcare services nearer to the patients, irrespective of distance and location constraints. After receiving professional cardiology advice, implementation of telemedicine program, such as tele-electrocardiography (tele-ECG) at the primary care level, may be a financially advantageous way to identify cardiovascular disease in the general population and avoid overtreating patients. This is our first time adopting tele-ECG consultations in East Indonesia under the Makassar Telemedicine Program. This program allows us to maintain a big database of cohorts and connect its implementation to real-world clinical practices, and at the end, could guiding the health workers to improve patient’s outcomes.

Development of a Mini-Mobile Digital Radiography System by Using Wireless Smart Devices

The current technologies that trend in digital radiology (DR) are toward systems using portable smart mobile as patient-centered care. We aimed to develop a mini-mobile DR system by using smart devices for wireless connection into medical information systems. We developed a minimobile DR system consisting of an X-ray source and a Complementary Metal-Oxide Semiconductor (CMOS) sensor based on a flat panel detector for small-field diagnostics in patients. It is used instead of the systems that are difficult to perform with a fixed traditional device. We also designed a method for embedded systems in the development of portable DR systems. The external interface used the fast and stable IEEE 802.11n wireless protocol, and we adapted the device for connections with Picture Archiving and Communication System (PACS) and smart devices. The smart device could display images on an external monitor other than the monitor in the DR system. The communication modules, main control board, and external interface supporting smart devices were implemented. Further, a smart viewer based on the external interface was developed to display image files on various smart devices. In addition, the advantage of operators is to reduce radiation dose when using remote smart devices. It is integrated with smart devices that can provide X-ray imaging services anywhere. With this technology, it can permit image observation on a smart device from a remote location by connecting to the external interface. We evaluated the response time of the mini-mobile DR system to compare to mobile PACS. The experimental results show that our system outperforms conventional mobile PACS in this regard.
