Equity of urban medical services affects human health and well-being in cities and is important in building ‘just’ cities. We carried out a quantitative analysis of the spatial accessibility of medical services considering the diverse demands of people of different ages, using outpatient appointment big data and refining the two-step floating catchment area (2SFCA) method. We used the traditional 2SFCA method to evaluate the overall spatial accessibility of medical services of 504 communities in Xiamen city, considering the total population and the supply of medical resources. Approximately half the communities had good access to medical services. The communities with high accessibility were mainly on Xiamen Island, and those with low accessibility were further from the central city. The refined 2SFCA method showed a more diverse and complex spatial distribution of accessibility to medical services. Overall, 209 communities had high accessibility to internal medicine services, 133 to surgery services, 50 to gynecology and obstetrics services, and 18 to pediatric services. The traditional method may over-evaluate or under-evaluate the accessibility of different types of medical services for most communities compared with the refined evaluation method. Our study can provide more precise information on urban medical service spatial accessibility to support just city development and design.

Identifying Areas of Low-Access to the COVID-19 Vaccine: A New Objective Framework Incorporating Mobility Data

Background Methods have been proposed to identify areas of low access to resources that are embedded with subjective parameters. In this work, we propose and validate a new Mobility Data-Driven (MDD) framework to identify areas that have low access to the COVID-19 vaccine. Methods We collected geospatial mobility data to an objective approach for determining areas of low access. We identify census tracts in Oregon with low access to the COVID-19 vaccine through two approaches: (1) An adapted United States Department of Agriculture (USDA) food desert definition and (2) our proposed MDD framework. Ten spatial and social measures of access are utilized to compare these two approaches. Results Tracts identified by the MDD definition have lower spatial accessibility scores (0.072 – 0.162) than those identified by the USDA adapted definition (0.239 – 0.32). During the Spring season, the MDD identified census tracts have a higher rate of poverty (15.2%), unemployment (8.4%), uninsured individuals (7.1%), and a lower per capita income ($28,261). Moreover, we find that the proportion the American Indian and Alaskan Native population in MDD identified low-access census tracts is higher than that in USDA definition (4.85% versus 0.95%) which indicates the framework’s ability to capture known disparities in access amongst this population. Conclusions During the creation of plans for equitable distribution of a resource like the COVID-19 vaccine, leaders should utilize objective data (like mobility data) to assist them in determining parameters that represent a populations ability to obtain that resource. Our proposed framework provides a starting point for achieving this goal.

With the rapid development of social economy in Chengdu, it is of great significance to analyze the accessibility of a public facilities resources, and services in respective regions for the spatial planning of public facilities and the optimization of urban construction in Chengdu. Most existing studies only consider the accessibility of a single public facility under a single transportation mode studying facilities at the same level. In this research, with the consideration of all these factors mentioned above, an improved 3‐step floating catchment area (3SFCA) model of multi‐transportation modes is introduced to analyze the accessibility and spatial distribution of multiple types of public facilities under multiple traffic modes. The new model classifies the grade of public facilities and calculates the service accessibility of public facilities under the driving, public transport, cycling, and walking modes in the five dimensions of education, medical care, commerce, transportation, and leisure in the central urban areas of Chengdu. Through the comparative analysis of multiple models, we found that the improved 3SFCA can more accurately evaluate the accessibility of all dimensions of public facilities in central urban areas of Chengdu. The accessibility of compound public facilities in the central city is excellent. With the gradual expansion to the outward areas, the diversity and accessibility of compound public facilities gradually decreases. Furthermore, the accessibility distribution of public facilities in the study area is not balanced. The accessibility between the central urban areas and the marginal areas differs greatly and the spatial differentiation is substantial.
