US in the diagnosis of gastroesophageal reflux in children

Abstract: Several techniques have been used to diagnose gastroesophageal reflux (GER) in children, but no single test is sufficiently accurate to completely investigate the problem. Gastroesophageal US has been described as a widely available, noninvasive and sensitive method. It provides morphological and functional information, but its role in the diagnosis of GER in children is still debated. In this paper we review diagnostic approaches to GER in children. We focus on current use of US in the management of children … Show more

“…The role of delayed gastric emptying in GER(D) remains controversial. Ultrasound provides morphological and functional data with high sensitivity and positive predictive value for the diagnosis of GER [102]. Sonographic assessment of findings such as abdominal esophageal length, esophageal diameter, esophageal wall thickness, and gastroesophageal angle provide important diagnostic indicators of reflux and related to the degree of GER [102].…”

“…The BS, the oldest diagnostic method used in this category, can evaluate the mu- cosal alterations and the anatomical abnormalities [22]; additionally, it can provide a good representation for classifying the severity of reflux [26,27].…”

“…Symptoms and signs associated with GERD are nonspecific, hence a clinical diagnosis of GERD cannot be used in infants and children who cannot reliably express the quality and quantity of their symptoms [22]. As a result, diagnostic tests are needed to document the presence of GERD, to detect the complications, to establish a causal relationship between severity of GERD and symptoms, to evaluate the therapy, and to exclude other conditions [23].…”

“…In cases of GER we always continue the contrast examination to the ligament of Trietz to exclude distal pathology, including obstruction of pyloric channel, duodenal stenosis and intestinal malrotation a b (5). US of GEJ can also be considered in the evaluation of GER (25). In hiatal hernia the GEJ is situated in the thorax together with part of the stomach ( Fig.…”
