The main purpose of this research is to describe: (1) sentence type; (2) Unity sentence; and (3) sentence structure found in the class VI students elementary school in Buleleng Regency, Bali Province. The data source used to be the subject in this study is a class VI elementary school student in Buleleng Regency, Bali province. Meanwhile, the object is (1) sentence type; (2) unity sentence; and (3) sentence structure found in the class VI students elementary School in Buleleng Regency, Bali Province. The method used in collecting data is a document recording method. Collected data is analyzed with descriptive analysis. The results showed that: (1) The type of sentence found in the class VI elementary school students in Buleleng Regency, Bali Province can be divided according to (a) the contents, there are 219 sentences; (b) The number of the clauses, there are 74 single sentences, 145 compound sentences; (c) The predicate of the establishment, there are 216 verbal sentences and 3 nominal sentences; and (d) the nature of the actor's relationship, there are 201 active sentences and 18 passive sentences.(2) The unity of the sentence indicated by the existence of (a) the subject (S) and the predicate (P): 214 sentences are clearly categorized as the unity of the idea; (b) 1 sentence is unclear the unity of the idea. The sentence that the unity of the idea is not clearly caused by the absence of S and P, is only a description; (c) Without S or contains only P: 1 sentence, and (d) without P or contain only S: 3 sentences. (3) Sentence structure, both the singular and compound found in the class VI students of an elementary school in Bali province quite varied. Based on the results obtained, it is advisable for students to always pay attention to the type, unity, and sentence structure in writing an essay for the effectiveness of ideas submitted.

Different classification systems of UTI exist. Most widely used are those developed by the Centers for Disease Control and Prevention (CDC) [6], Infectious Diseases Society of America (IDSA) [7], European Society of Clinical Microbiology and Infectious Diseases (ESCMID) [8] as well as the U.S. Food and Drug Administration (FDA) [9, 10]. Current UTI guidelines frequently use the concept of uncomplicated and complicated UTI with a number of modifications (Figure 1). In 2011 the EAU/EAU Section of Infections in Urology proposed the ORENUC classification system based on the clinical presentation of the UTI, categorisation of risk factors and availability of appropriate antimicrobial therapy [11]. Figure 1-Concept of uncomplicated and complicated UTI The following classification of UTIs is adopted in the EAU Urological Infections Guidelines: Classification of UTI Uncomplicated UTIs Acute, sporadic or recurrent lower (uncomplicated cystitis) and/or upper (uncomplicated pyelonephritis) UTI, limited to non-pregnant, premenopausal women with no known anatomical and functional abnormalities within the urinary tract or comorbidities. Complicated UTIs All UTIs which are not defined as uncomplicated. Meaning in a narrower sense UTIs in a patient with an increased chance of a complicated course: i.e. all men, pregnant women, patients with anatomical or functional abnormalities of the urinary tract, indwelling urinary catheters, renal diseases, and/or with other concomitant immunocompromising diseases for example, diabetes. Recurrent UTIs Recurrences of uncomplicated and/or complicated UTIs, with a frequency of at least three UTIs/year or two UTIs in the last six months. Catheter-associated UTIs Catheter-associated urinary tract infection (CA-UTI) refers to UTIs occurring in a person whose urinary tract is currently catheterised or has been catheterised within the past 48 hours. Urosepsis A systemic, deleterious host response to infection originating from the urinary tract and/or male genital organs. Urosepsis is accompanied by signs of systemic inflammation, presence of symptoms of organ dysfunction and persistent hypotension associated with tissue anoxia. 3.2 Antimicrobial stewardship Antimicrobial stewardship programmes aim to optimise the outcome of prevention and treatment of infection whilst curbing overuse and misuse of antimicrobial agents [12-16]. Measures of success include regulating antimicrobial prescribing, and reduction in both the rate of healthcare associated infections such as Clostridium difficile and the emergence of resistant organisms [16]. In urology, antimicrobial stewardship programmes should include a series of measures to ensure rational, evidence based use of antimicrobials in the prevention and treatment of infections of the urinary tract and male accessory glands, as well as non-antimicrobial strategies. Background This study aimed to investigate the clinical outcomes and antibiotic susceptibilities of causative microorganisms in patients with obstructive pyelonephritis associated with ureteral stones (OPU). Methods This retrospective cohort study included female patients diagnosed with community-acquired acute pyelonephritis (APN) at a tertiary care hospital from 2008 to 2017. Cases of APN associated with obstruction of the upper urinary tract due to the presence of ureteral stones and cases of APN without complications were compared. Propensity score matching was used to adjust for heterogeneity within each group. Results Of the 588 female patients with community-acquired APN, 107 patients were diagnosed with OPU and 481 patients were diagnosed with uncomplicated APN. Propensity score matching revealed that Enterobacteriaceae strains isolated from OPU cases were more resistant to fluoroquinolones (51.9% vs. 16.0%, p < 0.001). Extended-spectrum β-lactamase was detected in 22.2% and 21.0% of the Enterobacteriaceae strains isolated from OPU cases and uncomplicated APN cases, respectively (p = 1.000). The treatment failure rate was similar in the OPU and uncomplicated APN groups (16.0% vs. 21.0%, p = 0.545). Conclusions Antibiotic treatment for patients with OPU may be empirically selected in accordance with the treatment protocol for general pyelonephritis. Clinicians should exercise caution before prescribing fluoroquinolones for the treatment of OPU.
