Ürogenital travmalar tüm travmaların %10'unu oluşturmaktadır. Bu travmaların önemli bir kısmını ise alt üriner sistem yaralanmaları oluşturmaktadır. Bu makalede alt üriner sistem yaralanmaları ile ilgili deneyimlerimizin paylaşılması amaçlandı. Gereç ve Yöntemler: 23 Mart 2011 ile 1 Nisan 2016 tarihleri arasında acil polikliniğine başvuran alt üriner sistem ve penis yaralanmaları hastalarının dosyaları retrospektif olarak hastanemiz otomasyon sistemi taranarak incelendi. Hastalar demografik özellikler, etyoloji, eşlik eden yaralanma ve cerrahi sonuçları açısından değerlendirildi. Bulgular: Belirtilen tarihler arasında acil polikliniğine 14 mesane, 38 üretra ve penis yaralanması olmak üzere toplam 52 hastanın başvurduğu görüldü. Mesane yaralanması olan 14 hastanın 10'u erkek iken dördü kadın idi. Üretra ve penis yaralanması olan tüm hastalar erkek iken bu hastaların 22'sinde üretra 14'ünde penis yaralanması, ikisinde ise hem penis hem üretra yaralanması mevcut idi. Kavernozum yaralanması olan hastaların 12'sinde cinsel ilişki nedeniyle penil fraktür gelişmişti. İkisinde ise düşme sonra penil fraktür gelişmiş olduğu görüldü. Sonuç: Acil servise travma nedeniyle başvuran hastalarda alt üriner sistem ve penis yaralanmaları da olabileceği göz önünde bulundurularak dikkatli olunmalıdır.

Purpose: Severe pelvic fractures are associated with genitourinary injuries, but the relationship between pelvic trauma and concomitant urethral injuries has yet to be elucidated. This study evaluated the incidence, mechanism, site, and extent of urethral injuries in male patients with pelvic fractures. methods: A retrospective cohort study was performed involving patients with urethral injuries accompanying pelvic fractures who visited Pusan National University Hospital from January 1, 2014 to December 31, 2019. Demographics, mechanisms of injury, clinical features of the urethral injuries, concomitant bladder injuries, methods of management, and the configuration of the pelvic fractures were analyzed. results: The final study population included 24 patients. The overall incidence of urethral injury with pelvic fracture was 2.6%, with the most common mechanism of urethral injury being traffic accidents (62.5%). Complete urethral disruption (16/24, 66.7%) was more common than partial urethral injuries (8/24, 33.3%), and unstable pelvic fractures were the most common type of pelvic fracture observed (70.8%). There was no definitive relationship between the extent of urethral injury and pelvic ring stability. Conclusions: The present study provides a 6-year retrospective review characterizing the incidence, mechanism, and clinical features of urethral injury-associated pelvic fractures. This study suggests that the possibility of urethral injury must be considered, especially in unstable pelvic fracture patients, and that treatment should be chosen based on the clinical findings.

Biodegradable drug-eluting urethral stent in limiting urethral stricture formation after urethral injury: An experimental study in rabbit

In this study, a reproducible urethral injury animal model was developed and the role of the biodegradable drug-eluting urethral stent in limiting urethral stricture formation after urethral injury was evaluated. A total of 22 rabbits were used, and 20 rabbits were randomly chosen to develop urethral injury animal model. Bulbar urethral injury was made by a self-designed explosion device in the 20 rabbits. The urethral injury animal model was then randomly assigned to 2 groups of 10 each, which received a treatment of biodegradable paclitaxel-eluting urethral stent or only end-to-end anastomosis. Other two rabbits served as normal control group. Stents were surgically implanted into the injured urethras of rabbits under direct vision. Reparative effects, including stent degradation, were evaluated by urethroscopy, retrograde urethrography, and histology at different intervals at weeks 4, 8, and 12. In stent-free group, 8 of 10 rabbits developed obvious urethral stricture which was demonstrated by urethroscopy and retrograde urethrography, while in biodegradable paclitaxel-eluting stent group, urethral stricture was absent in all animals (p < 0.05). Histological follow-up indicated that the drug-eluting stents can also minimize the inflammatory reactions and fibrosis formation compared with the stent-free groups. Scanning electron microscope demonstrated that the biodegradable drug-eluting stent can gradually degrade in 12 weeks. The biodegradable paclitaxel-eluting urethral stent is effective in limiting urethral stricture formation after urethral injury.
