Ureteral Reconstruction With Bowel Segments: Experience With Eight Patients in a Single Institute

Abstract: PurposeAlthough replacement of the ureter with a bowel segment is indicated for large ureteral defects, it is still a challenging technique for urologists. We present our experience and outcome of ureteral reconstruction using bowel segments.Materials and MethodsUreteral reconstruction with bowel segments was performed in eight patients in our institute between 1969 and 2009. We investigated the position and length of the ureteral defect and methods of reconstruction as well as the patients' backgrounds, posto… Show more

“…There is risk of fistula formation[ 6 7 8 ] and anastomotic leak. [ 1 ] However, none of such complications were encountered in our study which included five patients with prior history of radiation for pelvic malignancy. This might be due to use of proximal healthy ileum with minimal grossly visible radiation changes.…”

“…These defects are classified as simple or complex on the basis of length of the involved segment and associated nephropathy. [ 1 ] This classification is important because the surgical options for management of complex long-segment ureteral defects are limited. Use of ileal segment is indicated in these cases; however, the technique is challenging and outcomes need to be assessed in detail.…”

“…In the case of extensive ureteral resection, surgical reconstruction by means of end-to-end anastomoses remains a major issue. Various surgical techniques were employed such as the psoas hitch (Warwick and Worth, 1969 ), the Boari flap (Boari, 1894 ) and the downward mobilization of the respective kidney (Sullivan et al, 1982 ), as well as the replacement of the ureters with ileal or bladder tissue or even nephrectomy (del Pizzo et al, 1998 ; Corvin et al, 2004 ; Schlote et al, 2004 ; Wolff et al, 2011 ; Takeuchi et al, 2014 ). These surgical techniques were frequently accompanied by serious complications, ranging from mucus formation, stenosis, and infection to renal failure (Corvin et al, 2004 ; Schlote et al, 2004 ; Wolff et al, 2011 ).…”
