Uremic Pericarditis

Abstract: SUMMARYUremic pericarditis occurred in 41% of 83 patients admitted to the chronic dialysis program at the Peter Bent Brigham Hospital. In the vast majority of these patients the pericarditis was present before dialysis and cleared clinically after beginning therapy. Instances of pericarditis that developed during regular dialysis were associated with metabolic stress such as surgery or infection or inadequate dialysis. When the pericarditis failed to resolve the patients usually died with sepsis or had severe … Show more

“…Pericarditis was demonstrated in 40 of 125 patients with terminal uremia (32 %). This frequency of occurrence is in accordance with the findings of others (2,3,12). In five out of the total 40 cases of uremic pericarditis, life-threatening acute pericardial constriction due to cardiac tamponade developed.…”

“…In most cases uremic pericarditis provides an indication for hemodialysis; however, universal heparinization in connection with dialysis has been mentioned as a causative factor in the development of hemopericardium (1,2,3,8,9).…”

“…This fibrotic state often manifests as a rough granular surface with irregular, scattered adhesions between parietal and visceral pericardium in a “ bread and butter” pattern[ 8 , 11 ]. However, densely adherent pericarditis and gross pericardial thickening with organizing fibrinous pericarditis have been found at autopsy in cases of uraemic disease[ 12 ].…”

“…It was first described by Richard Bright in 1836. Its incidence in the early days of dialysis was high – 41% in a study of 83 patients in Peter Brigham Hospital [ 2 ] and 12.5% in another study of 125 patients from Denmark [ 3 ]. However, more recently, its incidence has decreased to about 5–20% in chronic hemodialysis patients [ 4 , 5 ], perhaps due to more efficient hemodialysis therapy.…”
