Uptake of Colon Capsule Endoscopy vs Colonoscopy for Screening Relatives of Patients With Colorectal Cancer

Abstract: In a prospective study, similar numbers of FDRs of patients with CRC assigned to undergo CCE or colonoscopy agreed to participate, but most preferred to undergo colonoscopy. CCE was as effective as colonoscopy in detecting significant lesions; it could be a valid rescue strategy for subjects who reject screening colonoscopy. ClinicalTrials.gov number: NCT01557101.

“…Another interesting study examined the uptake of PCCE-2 versus conventional colonoscopy for the screening of asymptomatic first degree relatives (FDRs) of CRC patients (20). Among the 120 eligible FDRs who were assigned to undergo colon capsule endoscopy (CCE), only 68 (56.6%) agreed to participate in the study.…”

“…Capsule excretion rates or complete colorectal observational rates have varied among studies. Using the European traditional bowel preparation of a large-volume PEG (4.5-6 L) followed by oral sodium phosphate booster (1.5-2.0 L) and with prokinetics such as domperidone, capsule excretion rates within approximately 10 h or battery life has been 70%-88%, 9,[11][12][13][14][15][16][17][18][19] except for studies by van Gossum (93%) 20 and by Sacher-Huvelin (95%). 21 If the data of all listed studies above are taken together, the capsule excretion rate would be 88% (1243/1407).…”

“…Overall adequate colorectal cleansing levels at CCE in previous studies have been 52%-94%. 3,6,12,[14][15][16][17][18][19][20][21]25,26 Although castor oil did not significantly interfere with colorectal cleansing levels in spite of reducing fluid and lavage loading, the regimen with castor oil showed an average cleansing level. Sensitivity and specificity of the second-generation CCE for subjects with at least one polyp with maximum size 6 mm or larger in previous studies have been 84%-94% and 64%-94%, respectively.…”

“…An advantage of the CCE is the possibility to perform the investigation in the patients' home [4]. Former individuals' experiences can be of value to future patients, especially if they have a choice between methods [7], as is the case in colorectal examination [2,6,[8][9][10][11][12]. Moreover, new technologies optimally go through an assessment in order to guide politicians and clinical practice, and such a comprehensive health technology assessment (HTA) contains benefits and harms including patient-based evidence [13].…”
