Upper gastrointestinal investigations before gastric banding

Abstract: Endoscopy and pH monitoring do not predict outcome for gastric banding and therefore have no relevance in the selection of patients for gastric banding. Patients with an incompetent LES shown by manometry had a higher reoperation rate. If this finding can be confirmed, patients with LES incompetence may need another intervention.

“…Many stress the importance of very close patient follow-up and education, some even introducing the notion of partnership between the patient and the bariatric team. Some authors attempt to select patients on the basis of preoperative factors, such as anatomic and functional studies of the upper GI tract [11,[33][34][35][36][37], age/sex/BMI of the patients, or their willingness to durably modify their eating habits and their lifestyle [38]. All published series of GB for morbid obesity include a cohort of patients, variable in size, with excellent results both in terms of weight loss, and overall quality of life.…”

“…But upper endoscopy fails to demonstrate the majority of small hiatal hernias in these patients preoperatively. [14] found in their study that endoscopy and pH monitoring do not predict outcome for gastric banding, and therefore stated that these have no relevance in the selection of patients for gastric banding. They felt that hiatal hernias are grossly underappreciated in patients with morbid obesity, due to the presence of a large distal oesophageal fat pad.…”

“…Baseline measurements were made with all side-holes positioned in the stomach; by means of the station pull-through method, the catheter was pulled back through the high-pressure zone of the gastroesophageal junction until it has reached the upper esophageal sphincter. The LES was considered mechanically incompetent, if the LESP was 6 mmHg or lower, the intra-abdominal LES length was 1 cm or shorter and/or the total LES length was 2 cm or shorter [15,16].…”
