Upper-Airway Stimulation for Obstructive Sleep Apnea

Abstract: O bstructive sleep apnea (OSA) is a common disorder that affects sleep and quality of life. Because many people with OSA cannot or will not comply with the standard treatment with a continuous positive airway pressure (CPAP) mask, alternative treatments have been sought. Upper-airway stimulation using unilateral stimulation of the hypoglossal nerve is a possible option. This multicenter, prospective, single-group, cohort trial and a follow-up randomized therapy-withdrawal trial were performed to evaluate the c… Show more

“…25 More recently, upper airway stimulation by an implantable device has been advocated as a treatment for moderate-to-severe OSA, but the reported results of a mean reduction in the AHI by about 52%, from 32.0 events/h at baseline to 15.3 events/h one year after surgery is significantly less than the results achieved with MMA. 42 Currently, there is one report that examined the comparative effectiveness of MMA and CPAP therapy for the treatment of severe OSA. Vicini et al 43 performed a randomized clinical trial comparing MMA and auto-titrating positive airway pressure (APAP) where all patients were evaluated in the home setting by a level 3 unattended sleep study.…”

“…Further long-term study is needed to determine if effectiveness and stimulation settings are stable over time; however, previous studies in adult patients have shown a persistent response. 10 In addition, implantation of additional pediatric e4…”

“…8 This device has been shown to be effective in neurotypical adults with moderate to severe OSA with an apnea hypopnea index (AHI) <50 events/hour and a BMI <32, and without circumferential airway collapse at the level of the velopharynx. 9,10 The device was recently approved for commercial use in adult patients meeting select criteria.…”

“…Today, the gold standard treatment for OSA is "continuous positive airway pressure" but nearly one third of the patients have difficulty using it. Alternative treatments include ENT, maxillofacial and bariatric surgery [2,3], stimulating device implantation [4] and oral appliance therapy [5]. A key aspect for alternative treatments consists in selecting the appropriate one in order to tackle specific anatomical issues of a given patient.…”
