With less than 40 cases in the English literature, liposarcomas of the upper aerodigestive tract are extremely rare. We present the first Scandinavian case: a 72-year-old male with progressive dysphagia and dysphonia. Tumour was located in the left aryepiglottic fold, 4-5 mm with normal epithelium. Tumour was excised macroscopically radical and histology showed myxoid liposarcoma, grade I. The patient received adjuvant radiotherapy. The gold standard is core needle or excisional biopsy. Main treatment remains surgery and the use of adjuvant radiotherapy is debatable. Liposarcomas have a high rate of recurrence and a tendency of de-differentiation over time; therefore, long-term follow-up is recommended. The diagnosis can only be made histologically and preferably with FISH probes. ARTICLE HISTORY

Primary liposarcoma in the pyriform sinus of the hypopharynx is extremely rare and shows a distinct male preponderance. The complete resection has been a choice of the treatment, however the rate of recurrence is relatively high. This report presents a pregnant case with well-differentiated liposarcoma of the hypopharynx. The recurrent tumor involving the pyriform sinus and aryepiglottic folds was removed by an intraoral approach using direct suspension microlaryngoscopy and a Holmium-YAG laser. The tumor size was 30 mm x 15 mm x 10 mm. An additional systemic examination showed no distant metastasis. The patient delivered a healthy female baby without any trouble. The well-differentiated type of liposarcoma frequently shows local recurrence; however it rarely shows lymph node or distant metastasis. A complete function preserving excision such as intraoral approach may therefore appear to be an adequate management plan, however careful follow-up of these patients at regular intervals is also imperative for detecting of recurrence.

Diagnosis: Liposarcoma of the hypopharynxLiposarcomas represent only a small percentage of head and neck sarcomas. In one series of 103 head and neck sarcomas, 5 patients were diagnosed as having liposarcoma. 1 The extremities and retroperitoneum account for 95% of liposarcoma cases, with head and neck liposarcomas representing a fraction of the remaining 5%. 2 Hypopharyngeal liposarcoma is an exceedingly rare entity, with fewer than 10 reported cases, to our knowledge. 3 The most common presenting symptoms are dysphagia, difficulty breathing, and stridor. The reported age range is 25 to 90 years, and most cases occur in men. 3 This sex distribution is similar to that described in cases of laryngeal liposarcoma, only 10% of which have developed in women. 3 A polypoid presentation is often described as classic for liposarcoma of the head and neck, although the percentage of cases with a polypoid presentation is difficult to quantify, as it is often not specified in case reports.Weiss and Goldblum 2 suggest 3 categories of liposarcoma: (1) well-differentiated liposarcoma/atypical lipomatous tumor, which has no metastatic potential but can dedifferentiate and acquire metastatic potential; (2) myxoid/round cell liposarcoma; and (3) pleomorphic liposarcoma. Liposarcomas of mixed type are uncommon. Most liposarcomas of the upper aerodigestive tract are well differentiated and low grade, although moderate-or high-grade tumors have been reported. 3 The term well-differentiated liposarcoma is synonomous with atypical lipoma and atypical lipomatous tumor. Although these 3 types are histologically identical, there is a preference to refer to neoplasms arising in the soft tissues of the extremities and trunk as atypical lipoma and atypical lipomatous tumors because they rarely undergo dedifferentiation as they are usually diagnosed earlier in their clinical course. 2 Therefore, these superficial tumors are virtually never fatal. 4 In contrast, the designation welldifferentiated liposarcoma is preferred for tumors that arise in the retroperitoneum, spermatic cord, mediastinum, and internal structures, because these tumors are difficult to treat adequately by virtue of their anatomical location and often their large size. They also have a poorer outcome because of local aggressive growth that may be lethal. 5 Late diagnosis and challenges with complete excision in deep sites allow more time for the tumors to dedifferentiate to aggressive and metastatic forms. 2 Well-differentiated liposarcoma is subclassified into 3 major histologic subtypes: lipomalike, sclerosing, and inflammatory. The inflammatory subtype is the rarest. 4 There is no difference in clinical behavior between subtypes. 2 Lipoblasts are the diagnostic cell type of liposarcoma. They may be recognized by multiple cytoplastic vacuoles scalloping a hyperchromatic nucleus (Figure 3). Abstract: The literature supports that UADT liposarcomas are rare and usually of low-grade histologic type. The rate of metastatic disease and tumor-related mortality is low. However, high recurrence rates have been noted, particularly when less radical surgery is employed.
