Update on the management of acute pancreatitis

Abstract: Purpose of reviewThis review provides insight into the recent advancements in the management of acute pancreatitis.Recent findingsModerate fluid resuscitation and Ringer's lactate has advantages above aggressive fluid resuscitation and normal saline, respectively. “…Severe acute pancreatitis can lead to complications such as sepsis or multiorgan failure(10). But current treatments are mainly supportive care, including intravenous fluid administration, antibiotics to prevent further infection, and pain-killer for patient’s pain management(11). Surgery is utilized to remove damaged pancreatic tissue or obstructive gallstone(12, 13).…”

“…The management of acute pancreatitis, which is centered around fluid resuscitation, nutrition, and pain management, is constantly improving. The authors clearly describe the current trend towards a more conservative approach, which involves using fewer fluids, less opiates, and delaying interventions as much as possible [14]. Approaches that used probiotics or immunomodulation have so far not demonstrate any meaningful effect on clinical important outcomes in patients with severe pancreatitis.…”

Van den Berg et al [14] conclude that first we need a more fundamental understanding of the role of the gut, the microbiome and the early inflammatory response in pancreatitis.…”
