Update on outcomes research for cleft lip and palate

Abstract: The literature lacks any evidence-based consensus to support a superior method of cleft lip repair. A majority of North American surgeons, however, utilize a rotation-advancement technique and perform cleft rhinoplasty at the time of primary lip repair, with the idea that this could decrease the number of revision surgeries needed over the long term. Most cleft surgeons perform a single-stage palatoplasty at 9-12 months of age for improved early speech outcomes. There is insufficient evidence to support a two-… Show more

“…Outcomes are most typically measured through physician‐centered means that focus on function, aesthetics, and rate of complications, but patient quality‐of‐life (QOL) outcome measures offer a different method of determining successful treatment. Patient QOL outcome measures include self‐image, integration in society, functional status, and speech and aesthetic perceptions (Munz et al, ; Shaye, ). Additionally, studies using a sample population and health utility assessment scores have equated the perceived level of disability associated with being born with CL/P or CP as similar to being born with monocular blindness (Sinno et al, ).…”

“…Genetic and environmental factors speci c to each population could in uence these differences, requiring further exploration. [39]. In most cases, the surgical approach is entirely based on the personal preference of the surgeon, and most surgical centers have their own technique of choice [40].…”

“…There are advocates for cleft lip repair over a range of time frames, from the neonatal period to 5 to 6 months of age 19,20 (Level V evidence). Intrauterine repair has been piloted using animal models based on the potential benefit of no scar formation 21,22 (Level V evidence); however, this has not been seriously pursued in humans, as the theoretic benefits do not outweigh the risks of exposing both the mother and fetus to this procedure.…”
