Update in Thyroid Fine Needle Aspiration

Abstract: Thyroid fine needle aspiration (FNA) is a safe, cost effective, and relatively accurate method for guiding the initial management of a thyroid nodule. The popularity of thyroid FNA is reflected in the fact that over 350,000 thyroid FNAs are performed each year in the USA. As we move into the next decade, several issues pertaining to thyroid FNA are being addressed including: how to better apply thyroid FNA as a differential test for follicular-patterned thyroid tumors, how to manage the atypical thyroid FNA, a… Show more

“…Thyroid fine-needle aspiration (FNA) is still among the most commonly performed procedures to evaluate thyroid parenchymal nodules (1). Beside clinically evident and palpable thyroid nodules that are present in up to 7% of the general population, ultrasonographic evaluation of the neck performed in various clinical settings bring to light up to 70% of non-palpable and deep-seated nodules (2,3,4).…”

“…Among these, papillary thyroid carcinoma (PTC) is the most frequent histologic type, comprising 70% to 85% of all thyroid malignancies . Over the past 3 decades, fine‐needle aspiration biopsy (FNAB) has developed as the most effective method for evaluation and triage of patients with thyroid nodules, partially explaining the increased incidence of thyroid cancer during that period . The ability to accurately diagnose PTC by FNAB is essential because it has major implications for subsequent patient management .…”

“…This implies a national rate for the UK as a whole of around 500 FNAs per million. In the USA Faquin et al 13 state that 350 000 thyroid FNAs are performed per year, implying a rate of approximately 1100 per million population. Thyroid FNA is therefore a commonly performed procedure.…”
