Unusual presentation of Q fever in a kidney‐pancreas transplant recipient

Abstract

Q fever is uncommon in solid organ transplant (SOT) recipients. We describe a case of granulomatous lung disease as an unusual presentation of chronic Q fever in a kidney‐pancreas transplant recipient.

| INTRODUC TI ONQ fever in solid organ transplant (SOT) recipients is a rare phenomenon with few cases described in the medical literature. 1 We describe a case of acute Q fever in a renal transplant recipient which presented as pneumonia with rapidly progressive respiratory failure and later evolved into suspected infective endocarditis. This presentation is unique in that the pneumonia required mechanical ventilation for respiratory failure, whereas most patients experience a self-limited pneumonia course.
