Resumo: A obstrução intestinal constitui complicação freqüente, de etiologia multifatorial, apresentação clínica variável e alta morbidade. Uma vez esgotados os recursos conservadores em casos específicos, a laparotomia exploradora é empregada para o diagnóstico final e tratamento em grande número de pacientes. Apesar do sucesso da via laparoscópica no manuseio de diversas afecções, a utilização desta via na abordagem inicial da obstrução do intestino delgado tem sido bastante limitada e alvo de numerosas críticas. Entretanto, o acúmulo de experiência com o método nos últimos anos, aliado ao avanço tecnológico e instrumental, têm permitido tratar número cada vez maior de pacientes obstruídos por meio do acesso laparoscópico. Assim, o surgimento de novos instrumentos como grampeadores laparoscópicos, pinças e trocáteres menos traumáticos ajudaram a tornar a videolaparoscopia factível e segura nestes pacientes. Neste artigo, os autores apresentam uma revisão sobre o papel da vídeo-cirurgia em casos selecionados de obstrução intestinal, ressaltando a contribuição dos métodos minimamente invasivos para o arsenal diagnóstico e terapêutico desta importante complicação. Descritores INTRODUÇÃOA obstrução intestinal é afecção freqüentemente encontrada em todo o mundo. Há grande diversidade entre os vários grupos de pacientes e civilizações, diferentes espectros de apresentação clínica, bem como etiologias diversas, que possibilitam uma larga faixa de diagnósticos e modalidades terapêuticas. Apesar dos avanços contínuos, todos os métodos diagnósticos apresentam limitações e, muitas vezes, o próprio procedimento terapêutico tem função diagnóstica 1 . A videolaparoscopia (VL) diagnóstica oferece vantagens de ser um procedimento pouco invasivo e de alta acurácia. Além disso o desconforto, os riscos, e os custos ao paciente são relativamente menores, podendo evitar investigações diagnósticas caras, e possibilitar tratamento definitivo no mesmo procedimento 2,3 .Tradicionalmente, a laparotomia exploradora é considerada a modalidade terapêutica de escolha para a grande maioria dos pacientes que necessitam de abordagem cirúrgica. Porém, os avanços na técnica videolaparoscópica e o aperfeiçoamento do instrumental têm encorajado a aplicação da VL nas doenças do intestino delgado.

A small number of cases of small bowel obstruction caused by foods without the formation of phytobezoars have been reported. Repeated small bowel obstruction due to the ingestion of the same food is extremely rare. We present the case of 63-year-old woman who developed small bowel obstruction twice due to the ingestion of chestnuts without the formation of phytobezoars. This is the first reported case of repeated small bowel obstruction caused by chestnut ingestion. Careful interviews are necessary to determine the meal history of elderly patients and psychiatric patients.

Abstract-Geophagia, the intentional ingestion of clayey soil has both useful and negative health effects on humans due to the biomass, physicochemical, mineralogical and geochemical composition of the gritty material. In a few circumstances, clayey soil is viewed as a supplement to health deficiencies resulting from a less than stellar eating routine. Geophagy has been of interest to researchers and analysts due to continued habitual, religious and cultural practices by diverse communities globally in spite of reports contraindicating its purported benefits. The gritty material remains a thing of interest considering the nature in which they are obtained, transported and stored. Six geophagic clayey soil samples were obtained from chosen casual markets in South Africa. Four of these samples were originally from around South Africa and two were originally from Nigeria. The samples were analysed and characterised utilizing XRF, XRD, pH, and electrical conductivity to survey the nutritious qualities that support the continued use by mostly females and pregnant ladies in various parts of South Africa. Results of the XRD analysis revealed that the clay materials were mainly composed of kaolinite with minor palygorskite, illite, Amesite, Gupeite, Hematite and Magnetite. Quartz was the major non-clay constituent identified. Results of the XRF analysis showed average values of major elements such as SiO 2 (54.02%), Al 2 O 3 (35.45%), Fe 2 O 3 (6.73%), K 2 O (2.76%), MgO (1.16%) with MnO, Na 2 O and P 2 O 5 falling below 0.5%. Titanium was a major heavy metal identified from the samples which may result in heavy metal toxicity. The potential medicinal application of these clayey soils is supported by their kaolinite contents; in contrast the trace elements are pointers of probable adverse effects on humans.
