Unusual cause of acute abdomen in a child – torsion of greater omentum: report of two cases

Abstract: Omental torsion is very rare, and its diagnosis is usually made only after surgery. At laparoscopy, omental torsion is suspected when the appendix is normal and the symptoms and findings of torsion are present. Laparoscopy is a safe and effective approach for the diagnosis and management of omental torsion, with the advantages of reduced postoperative pain and hospital stay.

“…This disease is more common in well-nourished males and is related to obesity. There were 28 males and 4 females in this group, which is similar to literature [ 19 , 20 ]. According to previous studies, lesions mainly occur on the right side of the omentum, accounting for about 90% of all cases.…”

“…We analyzed the reason may be due to the randomness of the collected data, resulting in a difference result. The right abdomen has the presence of gallbladder, appendix, and other organs, often causing acute cholecystitis, appendicitis, etc., and the clinical symptoms of primary omental infarction are very similar to acute appendicitis and cholecystitis [ 20 ]. CT examination can provide multiplanar reconstruction to describe the anatomical location and origin of the mass as well as the relationship between the mass and adjacent organs and vascular system.…”

“…It is well known that acute appendicitis is one of the most common causes of right lower abdominal pain in pediatric patients, but not every pain in that area is caused by acute appendicitis [ 1 ]. Numerous conditions can simulate the clinical presentation of acute appendicitis, like acute gastroenteritis, mesenteric lymphadenitis, torsion of intra-abdominal organs, Meckel’s diverticulitis, Crohn’s disease, enterobiasis, etc [ 1 , 2 , 3 ]. Typical clinical appearance of acute appendicitis is present only in 30–40% of pediatric patients [ 4 , 5 ].…”

“…Laparoscopic appendectomy is superior to open appendectomy due to shorter hospitalization time, reduced postoperative analgesic requirement, early food tolerance, quicker return to work, and lower risk of wound infection [ 3 , 4 ]. Moreover, laparoscopy allows for greater visualization and identification of various abdominal disorders that might simulate acute appendicitis, especially if the vermiform appendix is found nonaltered [ 5 , 6 , 7 , 8 ]. Closing the appendiceal stump is a critical step in an appendectomy to avoid major complications, such as postoperative fistula, peritonitis, and sepsis [ 9 ].…”
