BackgroundThe Nationale Stillkommission was founded in Germany in 1994 to increase the acceptance of breastfeeding as the primary means of infant nutrition. Scientific studies like “Stillen und Säuglingsernährung (SuSe-Studie)”, and regional studies in Bavaria, Freiburg, Hamburg, and Berlin demonstrated breastfeeding initiation rates of 90 to 95%, but the total breastfeeding rate decreased to 25–61% after infants were 6 months old. One predictor of breastfeeding duration may be maternal motivation. The present study aimed to describe breastfeeding motivation.MethodsWe analysed data collected in 2004–2008, during a previous study, the Survey of Neonates in Pomerania (SNiP). We retrieved data regarding maternal breastfeeding motivation, family environment, and socioeconomic factors. We constructed a quantitative breastfeeding-motivation score to identify factors involved in maternal breastfeeding.ResultsNinety five percent of mothers who gave birth in the study period and area provided information in the survey. The breastfeeding initiation rate was 88.4%. Mothers’ intentions to provide exclusive breastfeeding (only breast milk, no other liquids or infant formula) increased linearly from 71.9% in 2005 to 76.8% in 2008. Women motivated to provide exclusive breastfeeding were, on average, older, primiparous, and able to deliver spontaneously more often than women with less breastfeeding motivation. Furthermore, women with no motivation to provide exclusive breastfeeding and women that intended to provide breastfeeding combined with a complementary nutrition source had visited prenatal classes less frequently, had lower levels education, had lower average incomes, had a German nationality more often, and used tobacco more often than women motivated to provide exclusive breastfeeding.ConclusionsBreastfeeding intentions increased during the SNiP Study. This study identified several factors that might serve for targeted breastfeeding promotion in mothers younger than 25 years, mothers with low education, and multiparous mothers or women who have received a caesarean section. Furthermore, breastfeeding motivation might be enhanced during pregnancy and/or after delivery by providing prenatal classes.

Ziel: Die vorliegende Arbeit hat zum Ziel, 20 Jahre nach Einrichtung der Nationalen Stillkommission anhand der in Deutschland publizierten Stilldaten einen Einblick in die Entwicklung des Stillverhaltens zu geben. Methodik: Zur Identifizierung relevanter Publikationen wurde eine umfassende Literaturrecherche in den Datenbanken PubMed und Web of Science mit den Suchbegriffen ?breast feeding" oder ?breastfeeding? in Kombination mit ?Germany? durchgef?hrt. Der Publikationszeitraum wurde daf?r auf die Jahre 1995?2014 begrenzt. Ergebnisse: Insgesamt wurden 35 Studien mit Daten zum Stillen f?r die Geburtsjahrg?nge 1990?2012 erfasst. Die Daten stammen mehrheitlich aus regionalen oder lokalen Erhebungen und sind ?berwiegend retrospektiv erhoben worden. Etwa 60% der Studien waren mit dem prim?ren Ziel durchgef?hrt worden, Daten zum Stillen bzw. zur Ern?hrung im S?uglingsalter zu erheben. Trotz hoher Stillraten zu Beginn (72?97%) ist nach wie vor in den ersten 2 Monaten der st?rkste Abfall der (Voll-)Stillraten zu verzeichnen, so dass bis zum Alter von 6 Monaten nur noch etwa 50% der S?uglinge ?berhaupt gestillt werden. Schlussfolgerung: Verst?rkte Stillf?rderma?nahmen und unter anderem der Ausbau Fr?her Hilfen zum Stillen sind notwendig, um den Stillerfolg in Deutschland nachhaltig zu sichern. Mit Blick auf die Defizite bei der Stilldatenerhebung erscheint die Errichtung eines effizienten standardisierten Stillmonitorings in Deutschland unverzichtbar.

NAM—help or burden? Intercultural evaluation of parental stress caused by nasoalveolar molding: a retrospective multi-center study

Objectives Nasoalveolar molding (NAM) was developed to facilitate easier treatment and better outcomes for cleft lip and palate (CLP) patients. The aim of this study was to investigate the parental burden and possible intercultural differences of this treatment modality, which is often argued to burden parents to an extraordinary amount. Materials and methods Standardized questionnaires (available in English, Mandarin, and German) with 15 non-specific and 14 NAM-specific items to be retrospectively answered by Likert scales by parents of unilateral CLP patients with completed NAM treatment. Results The parents of 117 patients from two treatment centers in Taiwan and Germany were included. A very high level of overall satisfaction was found in both countries with significant intercultural differences in prenatal parent information, feeding problems, dealing with 3rd party’s perception, and experienced personal effort. Conclusion NAM is an effective treatment tool for children’s CLP deformities and their caregivers in overcoming the feeling of helplessness. Intercultural differences may be due to infrastructural reasons, cultural attitudes and habits, or different public medical education. Clinical relevance In addition to facilitating easier surgical treatment, NAM can be seen as a powerful coping strategy for parents dealing with a CLP deformity of their child and does not seem to burden them extraordinarily.
