Unsuccessful mid-term results for distal humeral hemiepiphysiodesis to treat cubitus varus deformity in young children

Abstract: Cubitus varus is the most common complication following a pediatric humeral supracondylar fracture. No reports are available on the result of hemiepiphysiodesis to correct this deformity. We report the use of a transphyseal crossed cannulated screw (Metaizeau technique) in five very young children (mean 3 years and 7 months). No correction was observed after a mean of 3 years and 10 months of follow-up. The low growing capacity of the distal humeral physis makes corrective osteotomy the procedure of choice for… Show more

“…Despite the suboptimal results published in a recent series [ 18 ], we obtained a statistically significant correction of the deformity. It is true, however, that due to the lower growth potential of the distal humerus compared with the other anatomical regions, which accounts for 20% of humerus growth and 10% of limb growth [ 27 , 28 ], the annual correction rate is low, at 2.41° (1.9 to 3.2) for HUA and 1.51° (1.3 to 2.8) when measured by BA.…”

“…There are only three other published papers on guided growth of the lateral distal humerus: two case reports [ 16 , 17 ] and the recent short series of five patients published by Soldado et al [ 18 ] ( Table 3 ). The two case reports use the eight-Plate, as in our study, reporting good results.…”

“…Hence, this technique may better serve patients that are near skeletal maturity. Soldado et al [58] used transphyseal crossed cannulated screws (Metaizeau technique) to correct cubitus varus deformities in five very young children. The children had a mean age of 3 years and 7 months and were followed over a mean period of 3 years and 10 months.…”

“…Otros autores incluso han demostrado resultados fallidos a medio plazo de la hemiepifisiodesis humeral distal para tratar la deformidad en cúbito varo en niños pequeños. 108 Estos hallazgos estarían relacionados con que el húmero distal tiene una pequeña contribución al crecimiento longitudinal humeral, por lo que la desviación axial o la dismetría pueden no desarrollarse hasta mucho después de la lesión si es que ocurren en absoluto.…”
