Unrecognized Paroxysmal Supraventricular Tachycardia

Abstract: The clinical characteristics of patients with PSVT referred for electrophysiologically guided therapy can mimic panic disorder. Diagnosis of PSVT is often delayed by inappropriate rhythm detection techniques (Holter instead of event monitoring) and failure to recognize ventricular preexcitation on the sinus electrocardiogram; symptoms due to unrecognized PSVT are often ascribed to psychiatric conditions.

“…Such an abnormality would be expected to magnify sympathetically mediated responses, particularly emotionally driven responses in the heart where noradrenaline inactivation is so dependent on neuronal reuptake (33), perhaps causing sensitization to cardiac symptom development and predisposing to the development of PD. Cardiac patients with palpitations from recurrent supraventricular tachycardias (46) and those with implantable cardiac defibrillators (47), which are prone to discharge in error, certainly are prone to develop PD. Augmentation of the sympathetic neural signal in the heart, by impairment of neuronal noradrenaline reuptake, additionally could increase cardiac risk, predisposing to the development of cardiac tachyarrhythmia (13).…”

“…The symptoms of paroxysmal SVT may frequently mimic those of panic disorder. In a retrospective survey conducted by Lessmeier et al, 17 women were more likely than men to have their physician attribute their symptoms to panic, anxiety, or stress, thus delaying the correct diagnosis of SVT. Attempting to document rhythm during palpitations helped in determining whether an arrhythmia was truly present.…”

“…In some circumstances, patients with primary cardiac arrhythmias can be misdiagnosed as having panic disorder. 27 Accordingly, all patients were clinically assessed by a cardiologist (M.D.E.) participating in the study.…”
