Unrecognized Odontogenic Maxillary Sinusitis: A Cause of Endoscopic Sinus Surgery Failure

Abstract: Unrecognized periapical abscess is a cause of ESS failure and the radiological report frequently will fail to note the periapical infection. Dentists are unable to recognize periapical abscesses reliably with dental x-rays and exam. In patients with maxillary sinus disease, the teeth should be specifically examined as part of the radiological workup.

“…Recognition of OMS is important because failure to address the dental pathology can result in failure of medical and surgical therapies and persistence of symptoms. 8,9 We reviewed the English-language medical literature and CRS guidelines with attention to recognition and recommendations for OMS. Prospectively, we enrolled all patients in a 30-month period with OMS based on computed tomography (CT) scan and report on their clinical and demographic findings.…”

“…The changes were confirmed only by CB-CT scans. 17 Obayashi et alnoted that 71.3% of tooth infections related to the jaw were associated with changes in the maxillary sinus. 16 What is important, in most cases, the clinical tests ruled out the presence of odontogenic infections.…”

“…Unilateral maxillary sinusitis poses a significant challenge to otorhinolayngologists as it is multifactorial and less well recognized causes like odontogenic factor is implicated in etiology which if not recognized leads to failure of treatment. 4 Odontogenic source should be considered with history of dental pain or dentoalveolar surgery and those with extended unilateral sinusitis or unilateral sinusitis resistant to conventional treatment. 5 Diagnostic nasal endoscopy with CT improves the diagnostic accuracy.…”
