Unilateral One Stage Nasolabial Flap for Reconstruction of the Lips

Abstract: One stage reconstruction of lateral lower lip defects with/without commissural involvement in full or partial thickness defects is possible by NLF. Supplementary flaps are needed when the vermilion needs simultaneous reconstruction.

“…Rotation flap: The pivot point is the base of the NLF and the lateral limit of the defect coincides with the medial margin of the flap. This flap has a sole indication: reconstruction of the upper lip lateral segment [34], [35]. Transposition flap: This type of NLF is a variant of rotation flaps.…”

“…Different modifications of the nasolabial flaps and various combinations with other regional flaps have been described in the past literature [21,22] for better cosmetic and functional outcomes of the lower lip. Although the inclusion of the facial artery in the flap design is not essential for flap survival, ipsilateral facial artery preservation does increase flap reliability [6][7][8]. In all our cases, we have tried to preserve the facial vessels, although facial vain was ligated in 2 patients due to the intraoperative injury.…”

“…Amongst them, the nasolabial flap is considered a unique flap for the reconstruction of the lower lip, providing adequate functional and aesthetic results. Nasolabial flap (NLF) is an arterialized flap that may be superiorly based, inferiorly based [6], or centrally based [7], depending upon the arterial pedicle, although many authors have claimed a random blood supply without any definitive arterial pedicle. First described by Sushruta in 600 BC, [7] this flap has been used in the reconstruction of the floor of mouth, palate, tongue, lips, and tip and ala of the nose [1-3, 6, 8].…”

“…It is a simple flap that uses the skin reservoir lateral to the nasolabial fold for facial and oral cavity reconstruction. The flap pedicle can be superiorly, inferiorly, or centrally designed, and the flap has a wide range of motions including advancement, rotation, and transposition ( 1 , 2 ). Its usage in trauma surgery is rarely mentioned.…”
