ABSTRACTaIM: Minimally invasive surgical techniques for spine surgery have gained popularity over the last decade. In this study, the authors describe a unilateral dynamic stabilization for unilateral spinal pathologies. MaterIaL and MetHOds:A total of 7 patients with an average age of 46.1 years (range 21-80 years) were included in this study. They are represented various spinal pathologies (degenerative disorders, various tumor pathologies) and having undergone a unilateral facetectomy (or facetectomies) with or without a hemilaminectomy procedure were analyzed in this study. The surgical stabilization was achieved with unilateral dynamic stabilization. resuLts:The new surgical concept is described here, and the comparative early results of the Oswestry disability index (ODI) and the visual analog scale (VAS) are presented as well. The preoperative average ODI was 73.14 (range 62-94), and the average VAS was 8.14 (range 7-10). Twelve months after the surgery, ODI was 10.6 (range 0-16), and the average VAS was 1 (range 0-2) (p< 0.01).cOncLusIOn: Although the limited number of cases analyzed reflects data of only early patient outcome, the procedure is safe, feasible, and cost effective. We aimed to compare the effect of minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) and Crenel lateral interbody fusion (CLIF) on single segmental lumbar degenerative disease. Patients with single segmental lumbar degenerative disease undergoing MIS-TLIF (n = 28) and CLIF (n = 28) were enrolled from April to October 2017. Preoperative medical history, anthropometric data, and clinical data were recorded. Visual analogue scores and Oswestry disability index (ODI) were assessed. Radiography was performed before and after surgery. X-ray films were evaluated according to the Bridwell method, visual analogue scores and ODI scores were evaluated. There were no significant differences in the gender, age, clinical diagnosis, involved segment or preoperative ODI score between 2 groups (P > .05). During 12-month follow-up, MIS-TLIF group had less intraoperative blood loss, drainage, postoperative bedridden time, and hospital stay (P < .05), but more operation time and radiation exposure time compared with CLIF group (P < .05). CLIF group reported less pain than MIS-TLIF group (P > .05). Both groups had similar lumbar fusion rate (P > .05). Overall, CLIF has less complications, less trauma and faster recovery for the treatment of single segmental lumbar degenerate disease when compared with MIS-TLIF. Evaluation of more patients and longterm follow-up are still needed to further validate our findings.

To assess the short-term outcome of unilateral pedicle screw fixed intervertebral fusion surgery for degenerative disc disease via MAST QUADRANT minimally invasive system. Thirty patients with degenerative lumbar disease from September 2009 to September 2011 were enrolled and divided into two groups: (1) unilateral pedicle screw fixed intervertebral fusion via MAST QUADRANT system; and (2) conventional bilateral pedicle fixed intervertebral fusion via open surgery. The perioperative parameters (operation time, intraoperative blood loss, and length of stay) and the follow-up clinical curative effect parameters (VAS and ODI) were compared between two groups. All patients successfully completed surgery and were followed up for more than 1 year. The perioperative parameters of MAST QUADRANT system group were significantly better than the control group (P < 0.05). And the clinical curative effects were similar to conventional surgery (P > 0.05). Unilateral pedicle screw fixed intervertebral fusion via MAST QUADRANT system is a less invasive and safer surgical techniques. It offers a quick recovery and effective surgical option in the treatment of lumbar degenerative diseases.
