Unfavourable pattern of metastases in M0 breast cancer patients during 1978-2008: a population-based analysis of the Munich Cancer Registry

Abstract: Key words: metastases pattern, time to metastases, survival, breast cancer 2 ABSTRACT PurposeLittle is known about time trends in metastases in patients treated in routine health care facilities without metastases at diagnosis (M0) and about survival after these metastases. Methods Data ConclusionsIn the period 1978-2008, unfavourable changes in the pattern of metastases were exhibited and no improvement was observed in survival of patients after occurrence of metastases. An explanation might be the increased… Show more

“…The oldest cohort from 1978 to 1984 showed the best survival with the lowest prior exposure to adjuvant treatment. In a cooperation between the Munich Cancer Registry and Leiden University, these results were confirmed for the interval between 1978 and 2008 [19], i.e. unfavorable changes in the pattern of metastases, and, again, no improvements in survival of the patients after occurrence of metastases.…”

“…An explanation for these unsatisfying results may be that the increased use of adjuvant systemic treatment has less effect on the highly lethal liver and central nervous system (CNS) metastases than on bone metastases. The multivariate analyses revealed a 20% higher hazard (p = 0.02) of dying within 5 years following metastases in the last (2003-2008) vs. the first (1978-1983) cohort [19]. However, adjuvant systemic treatment contributed decisively to the overall prevention of metastases and, therefore, generally improved OS of breast cancer patients.…”

“…The potential risk of scalp cooling lies in the protection of scalp metastasis from the effects of chemotherapy. However, that potential risk has, to date, been proved in only 2 patients 5 . Both of those patients had hematologic malignancies which, based on the published report, are regarded as a contraindication to scalp cooling.…”

“…Van de Hurk and colleagues [ 4 ] published a 30-year follow-up study with more than 33,000 patients, in which 60 % of breast cancer patients developed metastasis to distant organs with the central nervous system being one of the sites with higher incidence. Moreover, Patricia Steeg and colleagues [ 5 ] showed in a study of more than 1000 breast cancer patients that 15 % developed a single metastasis in the brain.…”
