Understanding traumatic blunt cardiac injury

Abstract: Cardiac injuries are classified as blunt and penetrating injuries. In both the injuries, the major issue is missing the diagnosis and high mortality. Blunt cardiac injuries (BCI) are much more common than penetrating injuries. Aiming at a better understanding of BCI, we searched the literature from January 1847 to January 2012 by using MEDLINE and EMBASE search engines. Using the key word "Blunt Cardiac Injury," we found 1814 articles; out of which 716 articles were relevant. Herein, we review the causes, diag… Show more

“…Blunt cardiac injury is generally diagnosed while considering the results of different tests; however, its rupture must be diagnosed as soon as possible. In particular, blunt cardiac rupture is rare, except in high‐energy trauma cases . Therefore, diagnosis was challenging in the present case, highlighting the fact that trauma severity does not necessarily indicate the degree of blunt cardiac injury.…”

“…In particular, blunt cardiac rupture is rare, except in high-energy trauma cases. 1 Therefore, diagnosis was challenging in the present case, highlighting the fact that trauma severity does not necessarily indicate the degree of blunt cardiac injury. In our case, two possible mechanisms of right ventricular rupture were suspected, which was caused by compression between the sternum or the costal cartilages and spine, or by significant elevation of venous pressure by an intensive blow to thorax on squatting during playing.…”

“…The management and outcome of TCI are often dictated by the mechanism of injury, hemodynamic status and clinical features 20 . Patients with BCI without features of cardiac tamponade and with minimal hemopericardium on CT scan may be treated nonoperatively by doing serial physical examination and echocardiography 21 . PCI patients often require surgery 12, 17.…”

“…The results of our study were similar to the results in literature. The incidence of cardiac injury resulting from blunt chest trauma in literature varies between 8% and 76% [10]. The incidence of cardiac injury was 50% in Skinner et al’s study [9].…”
