Understanding the concept of the therapeutic alliance in group treatment for alcohol and drug problems

Abstract: This article investigates how treatment factors are described by different client groups and by treatment staff. The material consists of interviews with clients (n = 81) and treatment staff (n = 18). The analysis focuses on two central themes -the importance of the treatment group and of the treatment staff, along with how these descriptions relate to the concept of the therapeutic alliance. The descriptions differ in parts between the client groups and between clients and staff. Clients as well as staff high… Show more

“…Previous research has hypothesized that contextual factors within a clinical environment, such as an empathic patient-practitioner relationship, can have a direct physiological response on the subject, which mediates analgesic mechanisms in itself [19]. Groups with stronger group cohesion, have been found to allow subjects within the group, to create change in an easier manner [20], which potentially explains the above result of this research.…”

“…One important aspect of treatment that appears to be a significant predictor of positive SUD treatment outcomes is a strong therapeutic alliance, especially when formed at the beginning of treatment (Janeiro et al., 2018). It is also vital that group members build an alliance with each other, as participants in SUD group therapy have stressed the importance of a sense of community, cohesion, safety, support, and friendship (Greiff & Skogens, 2019). Recent research suggests that beyond basic counseling skills such as active listening and empathy, strengthening bonds formed in the therapeutic environment also consists of facilitating trust and reducing power dynamics by broaching racial, ethnic, and cultural (REC) aspects during counseling with intention (Day‐Vines et al., 2007, 2020).…”
