Understanding the complexity of population health interventions: assessing intervention system theory (ISyT)

Abstract: Given their inherent complexity, we need a better understanding of what is happening inside the “black box” of population health interventions. The theory-driven intervention/evaluation paradigm is one approach to addressing this question. However, barriers related to semantic or practical issues stand in the way of its complete integration into evaluation designs. In this paper, we attempt to clarify how various theories, models and frameworks can contribute to developing a context-dependent theory, helping u… Show more

“…For example, the usual approach of dividing coordination context into legal, political, economic and social dimensions exemplifies a thematical categorisation for descriptive purposes, an entirely different thing from a theoretical approach. 88 A (multi)theoretical approach permits a more profound and multifaceted analysis of complex phenomena. For example, the TCE considers the intragovernmental coordination a constellation of exchanges between the MDAs within and across levels and policy areas (each with interests above and beyond their institutional mandate).…”

“…Health mediation thus corresponds to a complex health intervention 47 because of its contextual anchoring. 48 Indeed, health mediation practices are multifaceted 49 50 even though a joint intervention base exists. Health mediation has blurred boundaries in the healthcare system, torn between the community approach and the universalist paradigm, the biomedical and the social worlds.…”

“…This review underlines several characteristics of health mediation that articulate education and healthcare system navigation actions, along with actions of mobilisation, engagement, and collaboration of local actors among themselves and with the populations. Health mediation thus corresponds to a complex health intervention47 because of its contextual anchoring 48. Indeed, health mediation practices are multifaceted49 50 even though a joint intervention base exists.…”

“…The principles for effectiveness form a set of hypothesis that can be validated in the second phase of the study. Principles should eventually provide simple and pragmatic guidance for action [66] for effective communitybased obesity prevention across contexts [29]. This means identifying those (combinations of ) programme components that underly significant and/or structural impact in communities, for example because they set into motion a reinforcing feedback loop.…”
