Understanding factors influencing physical activity and exercise in lung cancer: a systematic review

Abstract: This systematic review has identified the volume of literature demonstrating that barriers and enablers to PA in lung cancer are multidimensional and span diverse factors. These include patient-level factors, such as symptoms, comorbidities, sedentary lifestyle, mood and fear, and environmental factors. These factors should be considered to identify and develop suitable interventions and clinical services in attempt to increase PA in patients with lung cancer.

“…We observed variation in the selection of BCTs according to the type of intervention, exercise and symptom self‐management, and consequently, the domains of the COM‐B model targeted. Use of BCTs to overcome known barriers and to maximise changes associated with successful participation varied. Exercise studies aimed to enable physical capability and motivation through instruction, practice, action planning, and feedback BCTs, supported by the provision of equipment and social support.…”

“…The acceptability of rehabilitation interventions to patients with thoracic cancer is influenced by many factors including, the stage of disease, symptoms, comorbidities, cancer treatment intensity and overall health status. These factors interact with the patient, carer and clinician's recognition of rehabilitation need and perceptions about the possible benefits, harms, and demands of proposed rehabilitation intervention(s) . Where benefit is perceived as being important and achievable, the acceptability of an intervention increases …”

“…Of significance is that, although exercise interventions have been developed for people with lung cancer (Edvardsen et al, 2014;Jones, Eves, Waner, & Joy, 2009;Quist et al, 2011), this has had little impact on clinical practice (Granger et al, 2017). Furthermore, few studies have been developed for those who are treated surgically (Granger et al, 2017).…”

“…Despite the numerous physical and psychosocial health benefits of physical activity, as few as 50% of adults and 10% of young people appear to meet the current recommendations for sufficient participation in physical activity . The reasons for insufficient participation in physical activity in the general population include competing time interests, attitudes and motivation and environmental factors, such as inclement weather …”
