The aim of this narrative review is to provide a comprehensive overview and to define the framework to investigate the factors that determine the postprandial experience. Based on a series of proof-of-concept studies and related information, we propose that the biological responses to a meal depend on the characteristics of the meal, primarily its palatability and composition, and the responsiveness of the guest, which may be influenced by multiple previous and concurrent conditioning factors. This information provides the scientific backbone to the development of personalized gastronomy.

Pain, defined as an unpleasant sensory or emotional experience from actual or potential tissue damage, it has been the subject of multiple investigations seeking to explain its pathophysiology, from their genetic and molecular bases until their physical and biological principles, in order to develop different therapeutic options that reduce or eradicate his presentation among the population. During recent decades, the use of complementary and alternative therapies has taken strength and gained popularity, being particularly useful in some specific groups of patients, like those who have chronic pain. These therapies are constituted by a wide and varied group of therapeutic interventions such as ayurvedic medicine, homeopathy, herbal, aromatherapy, among others; it is important that health professional know it and consider it as an option in the integral management of the pain. RESUMENEl dolor, definido como una experiencia sensorial o emocional desagradable de daño tisular real o potencial, ha sido motivo de múltiples investigaciones que buscan explicar su fisiopatología, desde sus bases genéticas y moleculares, hasta sus principios físicos y biológicos, con el fin de desarrollar diferentes opciones terapéuticas que permitan disminuir o erradicar su presentación entre la población. Durante las últimas décadas, el uso de las terapias complementarias y alternativas ha tomado fuerza y ganado popularidad, siendo particularmente útiles en algunos grupos específicos de pacientes, como aquellos que presentan dolor crónico oncológico. Estas terapias están constituidas por un amplio y variado grupo de intervenciones terapéuticas tales como medicina herbal, ayurvédica, homeopatía, aromaterapia, entre otras; es importante entonces que el personal de salud las conozca y las considere como una opción en el manejo integral del dolor.Palabras clave: Terapias complementarias. INTRODUCCIÓNLa Asociación Internacional para el Estudio del Dolor (IASP) define dolor como "una experiencia sensorial o emocional desagradable de daño tisular real o potencial, o una sensación descrita de forma similar a daño tisular" (1). El dolor crónico es aquel que dura más de 3 meses y ha sido catalogado como una enfermedad del cerebro, debido a los cambios significativos en la función, anatomía y en la química, que producen alteraciones fisiopatológicas en las rutas del dolor (1,2).Debido a su asociación con la gran mayoría de las enfermedades, la terapia del dolor se desarrolla de manera paralela al tratamiento de la enfermedad a la que se encuentre asociado (3). No obstante, el dolor crónico se puede asociar con fracaso terapéutico y con complicaciones como desórdenes del sueño, pérdida del apetito, fatiga crónica, dependencia a las drogas, estrés y depresión (4).En los últimos años se ha logrado aclarar que hay mecanismos genéticos implicados en este fenómeno, razón por la cual entre un ser humano y otro varía marcadamente laRecibido: 09-07-13. Central facilitation and modulation of incoming nociceptive signals play an important role in the perception of pain. Disruption in central pain processing is present in many chronic pain conditions and can influence responses to specific therapies. Thus, the ability to precisely describe the state of central pain processing has profound clinical significance in both prognosis and prediction. Because it is not practical to record neuronal firings directly in the human spinal cord, surrogate behavior tests become an important tool to assess the state of central pain processing. Dynamic QST is one such test, and can probe both the ascending facilitation and descending modulation of incoming nociceptive signals via TS and CPM, respectively. Due to the large between-individual variability in the sensitivity to noxious signals, standardized TS and CPM tests may not yield any meaningful data in up to 50% of the population due to floor or ceiling effects. We present methodologies to individualize TS and CPM so we can capture these measures in a broader range of individuals than previously possible. We have used these methods successfully in several studies at the lab, and data from one ongoing study will be presented to demonstrate feasibility and potential applications of the methods.
