At birth, the umbilical cord contains various types of thin vessels that are near and outside the umbilicus and separate from the umbilical arteries and vein. These vessels are regarded as the remnant "vitelline vessels" and are often called "umbilical vessels", although this terminology could lead to confusion with the true umbilical arteries and vein. No study has yet comprehensively examined these vessels using histological sections. Our examination of these vessels in 25 midterm fetuses (gestational age: 10-16 weeks) led to five major findings: (i) all specimens had umbilical branches of the inferior epigastric artery; (ii) 5 specimens had vitelline vein remnants; (iii) 4 specimens had a thin artery originating from the left hepatic artery that ran along the umbilical vein; (iv) 2 specimens had a so-called "para-umbilical vein" that was along the umbilical vein and reached the umbilicus; and (v) all specimens had lymphatic vessels originating from the umbilicus that ran caudally along the umbilical artery. The pelvic vein tributaries were well developed along the intra-abdominal umbilical artery, but did not reach the umbilicus. The lymphatic vessel was distinguished from the veins by an intraluminar cluster of lymphocytes attaching to the endothelium. The arterial branch in the umbilical cord did not accompany veins and lymphatic vessels, in contrast to the mother artery in the rectus abdominis. All these thin vessels seemed to be obliterated when the fibrous umbilical ring grew during late-term. The para-umbilical collateral vein in adults might develop outside the fibrous umbilical ring after birth.

Background: The study aimed to introduce a rib and costal cartilage harvesting surgery by transumbilical single-incision laparoscopy and evaluate its efficiency and safety. Methods: Patients who underwent rib and costal cartilage harvest under different approaches (direct open approach and transumbilical) were collected in this retrospective study. The differences in the pain scores [visual analog scale (VAS)], postoperative appearance, and complications were compared between the two groups at 1, 2, 3, and 7 days after surgery. In addition, based on the minimal clinically important difference and the generalized estimating equation, the differences were compared between the two groups in terms of the VAS score. Results: On postoperative day 1, the VAS scores of the direct open approach group and the transumbilical group were significantly different, that is, 7.29 and 6.10, respectively ( P < 0.001). Also, the generalized estimating equation results were different ( P < 0.001). An interaction was observed between different groups and days, that is, a statistical difference was observed in the VAS score between the two groups ( P < 0.001). In terms of aesthetics and complications, patients from the transumbilical group had no scars on the chest wall and no obvious postoperative complications. Conclusions: Transumbilical single-incision laparoscopic surgery is an innovative surgical approach for harvesting ribs and costal cartilage, which leaves no scars on the skin of the chest wall and has the advantages of slight postoperative pain, quick recovery, and fewer complications. This novel surgery is beneficial to patients with higher aesthetic requirements.

Zusammenfassung Einleitung Das Erscheinungsbild des Bauchnabels nach DIEP-Lappenplastik oder Abdominoplastik spielt eine wesentliche Rolle bei der ästhetischen Wahrnehmung (1). Obgleich der Nabel keine Funktion hat, besteht kein Zweifel an der Bedeutung, die die Nabelform für das Selbstwertgefühl der Patientinnen gerade nach einer Brustkrebserkrankung hat (2). In der vorliegenden Studie haben wir zwei der bevorzugten in der Literatur beschriebenen Techniken an 72 Patientinnen hinsichtlich des ästhetischen Ergebnisses, der Komplikationen und der Sensibilität verglichen: das kaudale Läppchen (kuppelförmige Form) und die querovale Form des Nabels. Patienten und Methode Zweiundsiebzig Patientinnen, die zwischen Januar 2016 und Juli 2018 eine DIEP-Lappenplastik zur Brustrekonstruktion erhielten, wurden in dieser Studie retrospektiv erfasst. Es wurden zwei Techniken zur Nabelrekonstruktion verglichen: die querovale Nabelform und die Umbilikoplastik unter Verwendung eines kaudalen Läppchens, welche in einer kuppelförmigen Form des Bauchnabels resultiert. Um die ästhetischen Ergebnisse zu vergleichen, wurde eine Evaluation durch die Patientinnen und eine Bewertung durch drei unabhängige Plastische Chirurg*innen mindestens 6 Monate postoperativ durchgeführt. Patientinnen und Chirurg*innen wurden gebeten, das allgemeine Erscheinungsbild des Nabels, einschließlich der Narbenbildung und der Form, mit einer Punktzahl zwischen 1 und 6 zu bewerten (1=Sehr gut, 2=gut, 3=befriedigend, 4=ausreichend, 5=mangelhaft, 6=ungenügend). Des Weiteren wurde die Sensibilität des Nabels durch Befragung der Patientinnen und das Auftreten von Wundheilungsstörungen untersucht. Ergebnisse Beide Techniken zeigten ähnliche Werte der ästhetischen Zufriedenheit (p=0,49) im Rahmen der Selbsteinschätzung durch die Patientinnen. Die Plastischen Chirurg*innen bewerteten die Technik mit kaudalem Läppchen als signifikant besser gegenüber der Nabelplastik mit querovaler Form (p=0,042). Beim kaudalen Läppchen traten mehr Wundheilungsstörungen auf (11,1%) im Vergleich zum querovalen Nabel. Dies war jedoch nicht signifikant (p=0,16). Eine operative Revision war nicht erforderlich. Beim Nabel mit kaudalem Läppchen zeigte sich eine Tendenz zur Verbesserung der Sensibilität (60% gegenüber 45%), die jedoch nicht signifikant war (p=0,19). Schlussfolgerung Die Patientenzufriedenheit zeigte ähnliche Ergebnisse für die beiden Methoden der Umbilikoplastik. Beide Techniken wurden im Durchschnitt mit einem guten Ergebnis bewertet. Andererseits bewerteten die Chirurg*innen die Umbilikoplastik mit kaudalem Läppchen als ästhetisch ansprechender.
