“…In malignant lymph nodes, this "color hilum" is lost or displaced while multiple vessels penetrate the nodal capsule and enter the lymph node, distributing anarchically. 1,16,19,29 As also suggested by other authors, 2 we find Doppler sonography particularly helpful in some indeterminate circumstances, improving the operator's confidence in suspecting malignancy. When a lymph node shows diffuse thickening of the cortex (ie, a lower than usual size of the echoic hilum), detecting hilar vascularization decreases our suspicion, whereas an abnormal vessel distribution prompts further investigation.…”

“…We do not inject sonographic contrast media to assess the nodal status of patients with melanoma. Some authors 2,19 have used these intravenous agents to increase the intranodal vascular signals in these patients, but we believe that the Doppler mode sensitivity of current scanners is sufficient, and its further intensification would not add relevant information (but would increase costs). There is some preliminary experience 20 on the use of contrast media to enhance gray scale images, showing the different perfusion behaviors of reactive and of metastatic lymph nodes.…”

“…Moreover, it is not easy to perform a dynamic study with MRI as the patients are usually not able to cooperate properly 3 . Ultrasound scanning has recently become an important diagnostic tool in dermatology because it is easy to use, completely safe and it provides important diagnostic information 9 . We believe that dermatologists should be familiar with anterior tibialis muscle herniation so that they can arrive at a proper diagnosis and make a surgical referral when they encounter positional subcutaneous nodules in the lower legs.…”

“…Owing to its increased sensitivity to flow, power Doppler sonography is valuable in low-flow states and when optimal Doppler angles can not be obtained. Longer segments of vessels and more individual vessels can be visualized with power Doppler US than with conventional color Doppler sonography [1,2,9]. According to the vascular pattern detected on PDUS, the lesions were classified as having peripheral, central, or mixed-type vascularity, similar to a categorization reported for breast tumors [10].…”

“…PDUS has recently gained attention as an additional color flow imaging technique that overcomes some of the limitations of CDUS which include angle dependence and difficulty in separating background noise from true flow in slow-flow states [9]. Owing to its increased sensitivity to flow, power Doppler sonography is valuable in low-flow states and when optimal Doppler angles can not be obtained.…”
