Objective. Sonography is being used with increasing frequency in the assessment of locoregional tumor spread in patients with melanoma. Nevertheless, to maximize its practical impact, sonography should be performed with state-of-the-art equipment, by specifically trained operators, and using a careful exploration technique and well-defined diagnostic criteria. In this "how I do it"-type article, we illustrate our practical approach to sonography of cutaneous melanoma. Methods. We first illustrate the basic and advanced technical requirements; then we describe our exploration methods and our image interpretation approach; and finally, we report on our use of sonography as a guidance tool for interventional procedures. Special emphasis is given to methodological and interpretative clues, tricks, and pitfalls. Results. Sonography can be used in the initial staging of patients with melanoma, particularly in the screening of patients scheduled for a sentinel lymph node biopsy procedure. Additionally, sonography can be used during patient follow-up to detect locoregional recurrence earlier than palpation. Conclusions. Sonography plays a growing role in the assessment of the superficial spread of melanoma. Nevertheless, state-of-the-art equipment and careful exploration by trained operators are necessary.

The term muscle herniation represents focal muscular protrusions through an acquired or congenital fascial defect. The anterior tibialis muscle is the most common site of herniation. Dynamic ultrasonography has become an important tool in dermatology for diagnosing suspected muscle herniation because it is a non-invasive, highly accurate, readily available and cost-effective imaging technique. We present here the case of a 21-year-old male Korean soldier who underwent dynamic ultrasonography to confirm the diagnosis of anterior tibialis muscle herniation.

Material and Method: Patients were histopathologically divided into two groups as benign and malignant skin tumors. The dimensions of the lesion, the contour characteristics, existence of pagoda echo, levels of invasion, the internal echogenicities and vascular patterns were investigated. We analysed velocity waveforms and determinated peak systolic velocity, end-diastolic velocity, resistive index, and pulsatility index. Results: Eighty patients (35 female, 45 male) with populo-nodular lesions on the surface of the cutaneous and subcutaneous tissues were prospectively enrolled in this study. A significant difference was noted between malign and benign skin lesion groups as for color and power Doppler ultrasonographic findings such as blood flow analysis and as for the gray scale ultrasonographic findings such as irregular contour, pagoda echo and hypoechoic echogenicity. Blood flow was observed in 33 patients. The existence of the flow was mostly observed in 92.2% of the malign skin lesions and in 33.3% of the benign lesions. The analysis of peak systolic velocity, end-diastolic velocity, resistive index, and pulsatility index between the study groups did not reveal a significant difference (p>0.005). Discussion: Doppler ultrasound is a non-invasive and powerful technique for the differentiation of benign and malignant skin lesions.
