Background: Management of patients in the critical care setting is crucial. The availability, the absence of ionizing radiation and the non invasive nature of chest ultrasonography (US) have currently increased its use in the up-to-date work-up of various pleuropulmonary abnormalities in the critical care setting. Objective: To evaluate the sensitivity, specificity and diagnostic accuracy of chest US for various pleuropulmonary abnormalities in intensive care unit (ICU) patients. Materials and methods: Ninety consecutive patients admitted in chest ICU with respiratory distress were assessed clinically and by chest radiography (CXR). They were suspected to have a provisional diagnosis of any of the following pathological entities: pneumonic consolidation, bronchogenic carcinoma, metastatic pulmonary nodules, pleural effusion, pneumothorax, hydropneumothorax and mesothelioma. These patients were scheduled for chest computed tomography (CT) and prospectively reviewed using chest US. The results of chest US were compared with these of chest CT for each encountered pathological entity using chest CT as the diagnostic standard of reference to subsequently calculate the sensitivity, specificity and diagnostic accuracy of chest US. Results: The sensitivity, specificity and diagnostic accuracy of chest US were 100%, 96% and 97% for pneumonic consolidation, 71%, 100% and 98% for bronchogenic carcinoma and 92%, 100% and 99% for pneumothorax respectively. The sensitivity, specificity and diagnostic accuracy of 100% for the rest of the included pathological entities were obtained. Conclusion: Chest ultrasonography has a considerable diagnostic performance for various pleuropulmonary pathological conditions that may be encountered in the ICU patients making it as an adjunct tool in the up-to-date work-up of the ICU setting.

International evidence-based recommendations for point-of-care lung ultrasound

Background: The purpose of this study is to provide evidence-based and expert consensus recommendations for lung ultrasound with focus on emergency and critical care settings. Methods: A multidisciplinary panel of 28 experts from eight countries was involved. Literature was reviewed from January 1966 to June 2011. Consensus members searched multiple databases including Pubmed, Medline, OVID, Embase, and others. The process used to develop these evidence-based recommendations involved two phases: determining the level of quality of evidence and developing the recommendation. The quality of evidence is assessed by the grading of recommendation, assessment, development, and evaluation (GRADE) method. However, the GRADE system does not enforce a specific method on how the panel should reach decisions during the consensus process. Our methodology committee decided to utilize the RAND appropriateness method for panel judgment and decisions/consensus. Results: Seventythree proposed statements were examined and discussed in three conferences held in Bologna, Pisa, and Rome. Each conference included two rounds of face-to-face modified Delphi technique. Anonymous panel voting followed each round. The panel did not reach an agreement and therefore did not adopt any recommendations for six statements. Weak/ conditional recommendations were made for 2 statements, and strong recommendations were made for the remaining 65 statements. The statements were then recategorized and grouped to their current format. Internal and external peer-review processes took place before submission of the recommendations. Updates will occur at least every 4 years or whenever significant major changes in evidence appear. Conclusions: This document reflects the overall results of the first consensus conference on ''point-of-care'' lung ultrasound. Statements were discussed and elaborated by experts who published the vast majority of papers on clinical use of lung ultrasound in the last 20 years. Recommendations were produced to guide implementation, development, and standardization of lung ultrasound in all relevant settings.

Az ultrahangvizsgálat jelentősége és fontossága a modern képalkotó vizsgálatok (CT, MR, izotóp, PET/CT) elterjedésével csökkent, nemritkán csak mint első tájékozódó vizsgálat jelenik meg. A legújabb sokszeletes spirál-CT-kkel a képalkotás másodpercek alatt elvégezhető, ami a jelentős sugárterheléssel járó vizsgálatok számának drámai növe-kedéséhez vezetett, gyermekkorban is. Ugyanakkor nem feledkezhetünk meg arról, hogy az elmúlt években jelentős fejlődés történt az ultrahang-technológiában, a képminőség egyre jobb. A gyermekek ideális alanyai a vizsgálatnak, miután az esetek nagy részében testtömegük kisebb, kevesebb zsírt tartalmaz, így könnyen vizsgálhatók magas frekvenciájú transzducerrel, aminek következtében sokkal részletgazdagabb, jobb felbontású képek nyerhetők, mint ál-talában felnőtteknél. Megfelelő készülék és gyakorlott, gyermekradiológiában tapasztalt vizsgáló esetében csaknem valamennyi testtájék vizsgálható, mindenképpen első választandó módszer, amely legtöbbször elegendő (esetleg röntgenvizsgálattal kiegészítve) a megfelelő diagnózis felállítására. In some cases, ultrasound is only the fi rst exploratory imaging method. Using the latest multi-slice CT, imaging can be performed in seconds, which led to a dramatic increase in the number of CT exams. However, this also means a signifi cant radiation exposure to children, while US still harmless in this regard. In addition, signifi cant progress has been made in ultrasound technology in recent years, which led an improvement in image quality. Children are ideal subjects for US examination as they usually have smaller weight with less body fat. Thus, ultrasound examination is easy to perform with a high frequency transducer resulting in much more detailed and higher resolution than in adults. With adequate equipment and experienced examiner in pediatric radiology, almost all parts of the body can be examined, making this technique as the fi rst (sometimes together with X-ray) and, in most cases, the ultimate imaging exam for the diagnosis. This article will discuss the possibilities where ultrasound performed with a modern device is suffi cient for an accurate diagnosis.
