Ultrasound-guided percutaneous thrombin injection for treatment of extrarenal pseudoaneurysm after renal transplantation.

Abstract: A patient who received two kidney transplants was placed on hemodialysis after failure of both grafts. He had complained of abdominal pain during the preceding month and was found to have a pulsating mass in his right pelvic region. Computed tomography and color Doppler studies showed a large pseudoaneurysm of the common iliac artery. We describe the treatment of this lesion by the use of ultrasound-guided thrombin injection. To the best of our knowledge, this is the first report regarding the use of ultrasoun… Show more

“…However, reports of successful endovascular techniques have been published with preservation of the graft function [1,3,13]. In the absence of infection, asymptomatic extrarenal pseudoaneurysms can be treated conservatively with strict observation [5].…”

“…The most significant risk is possible downstream arterial embolization. 23,24 This risk is reduced if the needle is carefully placed as far as possible from the neck of the pseudoaneurysm and if thrombin is slowly injected under continuous control by Doppler ultrasonography. 24 The second most significant risk is the possibility of allergy.…”

“…8 Case reports of ultrasound-guided percutaneous thrombin injection into the lumen of a pseudoaneurysm have provided evidence that thrombosis of the lumen of a femoral iatrogenic pseudoaneurysm is safe and efficient. [9][10][11] This technique's range of applications has recently increased, 9-11 yet we know of no published case reports on its use in the treatment of pseudoaneurysms in patients with Behçet's syndrome.…”
