In addition to hepatocellular carcinoma, metastatic liver cancer (MLC) is another focus of hepatic surgeon. Good outcome of patients with liver metastasis (LM) from colorectal cancer or neuroendocrine tumor have been achieved. Ovarian cancer liver metastasis (OCLM) has its unique oncological characteristics and a variety of metastasis patterns, which brings a challenge to hepatic surgeon. Hepatic surgeons hold different views and techniques from gynecologists, which makes differences in the evaluation and treatment of the disease. We reviewed recent studies and, in combination with our own clinical experience, attempted to introduce the progress of surgical treatment of liver metastases from OC. In our experience, both preoperative imaging and surgical procedures are based on the assurance of R0 resection. R0 cytoreductive surgery (CRS) is the most favorable determinant for the prognosis of OC patients, and R0 liver resection (LR) is a component of R0 CRS. Gynecologists and hepatic surgeons should do their own preoperative and intraoperative evaluation for the extrahepatic and intrahepatic metastasis respectively. During the operation, regardless of the miliary nodules dissemination between the right hemidiaphragm and liver capsule, liver parenchymal infiltration (LPI) or liver parenchymal metastasis (LPM), 1-2 cm resection margin should be emphasized. For patients with liver portal lymph node metastasis (LPLNM), hepatic portal skeletonization should be performed, rather than portal lymph node dissection. The operation should be as radical as possible to ensure the patients to achieve good prognosis.

Objective: To compare the oncological outcomes between microwave ablation (MWA) and surgical resection (SR) in patients with ovarian cancer liver metastasis (OCLM). Materials and methods: In this retrospective study, a total of 29 female patients (mean age, 47.8 ± 12.9 years; range, 21-65 years) diagnosed with forty-three OCLM nodules between September 2008 and July 2016 were included. All patients with ovarian cancer received chemotherapy and cytoreductive surgery (CRS). Fifteen patients with 22 nodules underwent MWA, and 14 patients with 21 nodules underwent SR. Overall survival (OS), local tumor recurrence-free survival (LTRS), and operationrelated parameters were compared between the two groups. Multivariate analyses were performed on clinicopathological variables to identify factors affecting OS and LTRS. Results: The median follow-up time was 70.2 months (range, 12.1-107.2 months). Fourteen patients died during this period. The 1-, 3-, and 5-year OS and LTRS rates after MWA were comparable to those after SR (p ¼ .198 and p ¼ .889, respectively). Compared with the SR group, the MWA group had a shorter surgical time (p < .001), less estimated blood loss (p < .001), shorter postoperative hospitalization (p < .001) and fewer costs (p ¼ .015). The multivariate analysis showed that old age (p ¼ .001) was a predictor of poor OS and that intrahepatic tumor size (p ¼ .005) and intrahepatic tumor number (p ¼ .001) were predictors of poor LTRS. Conclusion: Percutaneous MWA had comparable oncologic outcomes with those of SR and could be a safe and effective treatment for OCLM.

Background: Ovarian cancer is a heterogeneous and aggressive malignant tumor, and the liver is one of the most common metastases target visceral organs of ovarian cancer. We aim to analysis the incidence and prognostic relevance of histological subtypes for patients with liver metastases in newly diagnosed ovarian cancer. Methods: In the Surveillance, Epidemiology, and End Results (SEER) database, we identified the ovarian cancer patients from 2010 to 2016. Multivariable logistic regression was used to determine whether histological types were associated with the presence of liver metastases at diagnosis. The Kaplan-Meier method and multivariable Cox regression was performed to identify covariates associated with survival using the histological types.Results: Among 25293 ovarian cancer patients, 1749 cases presented with liver metastases. The incidence proportions were highest among ovarian carcinosarcoma patients (OR=17.76, 95% CI=9.26-34.09), and liver metastasis specificity was the highest in the clear cell type (70.69% of the metastatic subset). The median cancer-specific survival (CSS) for non-metastatic ovarian cancer patients was 77 months, but the ovarian cancer with only liver metastasis was 21 months. The mucinous (5 months; vs nonepithelial subtype, HR=0.26; 95% CI, 0.14-0.49) subtype experienced the shortest median survival among all histologic types.Conclusion: This population-based study provides that liver was one of the most common distant visceral organs for ovarian cancer metastasis, and the incidence proportions of liver metastasis were highest for carcinosarcomas subtype, and the mucinous ovarian cancer with liver metastasis being associated with the poorest survival.
