Although pain treatment has been described as a fundamental human right, the Coronavirus disease 2019 (COVID-19) pandemic forced healthcare systems worldwide to redistribute healthcare resources toward intensive care units and other COVID-19 dedicated sites. As most chronic pain services were subsequently deemed non-urgent, all outpatient and elective interventional procedures have been reduced or interrupted during the COVID-19 pandemic in order to reduce the risk of viral spread. The shutdown of pain services jointly to the home lockdown imposed by governments has affected chronic pain management worldwide with additional impact on patients' psychological health. Therefore, the aim of this review is to analyze the impact of COVID-19 pandemic on chronic pain treatment and to address what types of strategies can be implemented or supported in order to overcome imposed limitations in delivery of chronic pain patient care.

Consensus recommendations for botulinum toxin injections in the spasticity management of children with cerebral palsy during COVID-19 outbreak

Spasticity is the most common motor disturbance in cerebral palsy (CP). Lockdown in the COVID-19 outbreak has profoundly changed daily routines, and similarly caused the suspension of spasticity treatment plans. Besides, the delay in botulinum toxin (BoNT) injection, which is important in the management of focal spasticity, led to some problems in children. This consensus report includes BoNT injection recommendations in the management of spasticity during the COVID-19 pandemic in children with CP. In order to develop the consensus report, physical medicine and rehabilitation (PMR) specialists experienced in the field of pediatric rehabilitation and BoNT injections were invited by Pediatric Rehabilitation Association. Items were prepared and adapted to the Delphi technique by PMR specialists. Then they were asked to the physicians experienced in BoNT injections (PMR specialist, pediatric orthopedists, and pediatric neurologists) or COVID-19 (pediatric infectious disease, adult infectious disease). In conclusion, the experts agree that conservative management approaches for spasticity may be the initial steps before BoNT injections. BoNT injections can be administered to children with CP with appropriate indications and with necessary precautions during the pandemic.

Chest computed tomography (CT) may provide insights in the pathophysiology of COVID-19, although it is not suitable for a timely bedside dynamic assessment of patients admitted to intensive care unit (ICU); therefore, lung ultrasound (LUS) has been proposed as complementary diagnostic tool. The aims of this study were to investigate different lungs’ phenotypes in patients with COVID-19, assessing differences between ICU survivors and non-survivors in CT and LUS scores. We also explored the association between CT and LUS, and oxygenation (PaO 2 /FiO 2 ) and clinical parameters. Thirty-nine COVID-19 patients were included. CT scan revealed type 1, 2, 3 phenotypes in 62%, 28%, and 10% respectively. Among survivors, pattern-1 was prevalent (p<0.005). Chest CT and LUS scores differed between survivors and non-survivors both at ICU admission and 10-days thereafter and were associated with ICU-mortality. Chest CT score positively correlated with LUS findings at ICU admission (r=0.953, p<0.0001), while inversely correlated with PaO 2 /FiO 2 (r=-0.375, p=0.019), and C-Reactive Protein (r=0.329, p=0.041). LUS score inversely correlated with PaO 2 /FiO 2 (r=-0.345, p=0.031). COVID-19 presents distinct phenotypes, with differences between survivors and non-survivors. LUS should be considered as valuable monitoring tool in ICU setting, since it may correlate with CT findings and mortality, although it cannot predict oxygenation changes.
