Ultrasound‐guided injection of botulinum toxin in a patient with omohyoid muscle syndrome: A case report

Abstract: Omohyoid muscle syndrome has a characteristic feature of a protruding lateral neck mass during swallowing. We present a case of omohyoid muscle syndrome diagnosed based on the clinical presentation and a dynamic imaging study. Botulinum toxin was injected into the inferior belly of one omohyoid muscle, and the neck mass resolved.

“…We suspect that patients with sternohyoid syndrome demonstrate symptoms similar to omohyoid syndrome. Dynamic CT scan and realtime ultrasonography also provide useful information about strap muscles [8,13,15,16]. CT scan showed no abnormal finding of the sternohyoid and omohyoid muscles, which indicates that CT and ultrasound cannot be definite diagnostic tools of sternohyoid or omohyoid syndromes.…”

“…We also believe that education is important, as the condition is not cancerous. Su et al used botulinum toxin injection for spasmodic omohyoid muscle, as well [16]. In fact, the neck mass decreased completely and the foreign body sensation disappeared within three months after the injection.…”

“…The procedure leaves 5 cm or longer scar on the neck. Botulinum toxin injection to omohyoid muscle under ultrasonography guidance for OMS could offer an effect of omohyoid muscle dilation without operative scars on the neck, but it was not reported whether OMS would recur or if another injection was required (9). Theoretically, the paralysis of the degenereted omohyoid muscle caused by botulinum toxin couldn't be complete and the effect couldn't be long lasting.…”
